經(jīng)典三叉神經(jīng)痛患者靜息態(tài)腦功能局部一致性研究

【摘要】:目的:采用靜息態(tài)fMRI對經(jīng)典三叉神經(jīng)痛(CTN)患者的自發(fā)腦功能活動情況進(jìn)行研究,探究其局部一致性(ReHo)異常的腦區(qū)及其異常腦區(qū)的自發(fā)活動與個體病程和疼痛強(qiáng)度之間的關(guān)系,為進(jìn)一步了解CTN的中樞發(fā)病機(jī)制提供影像學(xué)新證據(jù)。研究方法:對25例CTN患者和25名健康對照者進(jìn)行靜息態(tài)下腦功能磁共振數(shù)據(jù)采集;利用DPARSFA軟件對影像數(shù)據(jù)行ReHo分析,然后用dpabi統(tǒng)計模塊對病例組及對照組的ReHo值進(jìn)行兩樣本t檢驗;對組間差異腦區(qū)ReHo值分別與患者的視覺模擬評分(VAS)和病程行相關(guān)分析。結(jié)果:與正常對照組相比,CTN患者ReHo值顯著減低的腦區(qū)包括右側(cè)腦干、右側(cè)額下回、左側(cè)丘腦、左側(cè)顳下回/梭狀回和左側(cè)邊緣葉/海馬旁回;患者ReHo值顯著升高的腦區(qū)包括左側(cè)中央后回、左側(cè)前扣帶回、左側(cè)頂下小葉和右側(cè)枕葉/楔葉/矩狀裂周圍皮層。右側(cè)腦干顯著減低的ReHo值與VAS呈正相關(guān);余組間差異腦區(qū)與VAS和病程均未發(fā)現(xiàn)相關(guān)性。結(jié)論:CTN患者存在疼痛相關(guān)的功能腦區(qū)局部一致性活動的異常,這些腦區(qū)與疼痛處理和運(yùn)動功能有關(guān)。這將有助于對CTN病理生理機(jī)制的理解。
[Abstract]:Objective: to study the spontaneous brain function in patients with classical trigeminal neuralgia (CTN) by resting fMRI, and to explore the relationship between the spontaneous activity of the brain region with local consistent (ReHo) abnormality and its abnormal brain area and the individual course of disease and the intensity of pain. To provide new imaging evidence for further understanding the central pathogenesis of CTN. Methods: 25 patients with CTN and 25 healthy controls were collected by resting functional magnetic resonance imaging (fMRI), and ReHo was used to analyze the image data with DPARSFA software. Then dpabi statistical module was used to carry out two samples t test on ReHo value of case group and control group, and correlation analysis was made between ReHo value of different brain area and visual analogue score (VAS) and course of disease. Results: compared with the control group, the areas of ReHo decreased significantly in patients with CTN included right brainstem, right inferior frontal gyrus, left thalamus, left infratemporal / fusiform gyrus and left marginal lobe / pariprahippocampal gyrus. Patients with significantly increased ReHo included left posterior central gyrus, left anterior cingulate gyrus, left inferior parietal lobule and right occipital / cuneiform / parietal cortex. There was a positive correlation between the ReHo value of the right brain stem and VAS, and there was no correlation between the difference of brain area and VAS and the course of disease in the other groups. Conclusion there is abnormal local consistent activity in functional brain regions related to pain in patients with 1: CTN. These areas are related to pain management and motor function. This will contribute to understanding the pathophysiological mechanism of CTN.
