【摘要】：目的:觀察腦卒中患者及伴有肩手綜合征、痙攣和肩關(guān)節(jié)半脫位的腦卒中患者的肩部皮膚溫度變化特點(diǎn)并探討紅外熱成像檢查是否可作為腦卒中后肩痛的客觀評(píng)價(jià)指標(biāo)。方法:選取86例腦卒中患者,使用數(shù)位式紅外線熱影像測(cè)溫系統(tǒng)對(duì)腦卒中患者,伴有肩手綜合征、痙攣和肩關(guān)節(jié)半脫位的腦卒中患者,以及腦卒中后肩痛患者進(jìn)行肩部紅外熱成像觀察。結(jié)果:70例(81.4%)患者患側(cè)肩部較健側(cè)溫度低,肩手綜合征與非肩手綜合征患者比較,差異有顯著性意義(P0.05);痙攣與非痙攣患者以及肩關(guān)節(jié)半脫位與非肩關(guān)節(jié)半脫位患者比較,差異均無顯著性意義(P0.05)。腦卒中肩痛與非肩痛患者比較,差異無顯著性意義(P0.05)。結(jié)論:紅外熱成像檢查能客觀反映腦卒中患者肩部皮膚溫度變化特點(diǎn),大部分患者的患側(cè)肩部皮膚溫度較健側(cè)低。合并肩手綜合征的患者兩側(cè)皮膚溫度不對(duì)稱現(xiàn)象更明顯,痙攣和肩關(guān)節(jié)半脫位不會(huì)明顯影響皮膚溫度。腦卒中后肩痛患者與非肩痛患者的紅外熱成像表現(xiàn)無明顯差異。
[Abstract]:Objective: to observe the changes of shoulder skin temperature in patients with stroke and those with shoulder and hand syndrome, spasm and subluxation of shoulder joint, and to explore whether infrared thermal imaging can be used as an objective evaluation index of shoulder pain after stroke. Methods: 86 patients with cerebral apoplexy were treated with digital infrared thermal imaging thermometry system, including patients with shoulder and hand syndrome, spasm and subluxation of shoulder joint. And the patients with shoulder pain after stroke were observed by infrared thermal imaging of shoulder. Results 70 cases (81.4%) had lower shoulder temperature than normal side, there was significant difference between shoulder and hand syndrome and non-shoulder hand syndrome (P0.05), spasm and non-spasm, shoulder subluxation and non-shoulder subluxation. There was no significant difference (P0.05). There was no significant difference between shoulder pain and non-shoulder pain in stroke patients (P0.05). Conclusion: infrared thermal imaging can objectively reflect the characteristics of shoulder skin temperature changes in stroke patients, and the skin temperature of most patients is lower than that of healthy side. Skin temperature asymmetry was more obvious in patients with shoulder-hand syndrome. Spasm and shoulder subluxation did not significantly affect skin temperature. There was no significant difference in infrared thermal imaging between patients with shoulder pain and those with non-shoulder pain after stroke.
