顳葉癲癇的磁共振CUBE成像技術(shù)臨床應(yīng)用價值

【摘要】:目的探討磁共振各向同性容積掃描(CUBE)成像技術(shù)在顳葉癲癇(TLE)腦內(nèi)病變檢測的臨床應(yīng)用價值。方法搜集2015年6月至2016年9月本院臨床診斷為癲癇患者30例,年齡7~65歲,平均31.7歲。均行MRI掃描,掃描序列包括矢狀位T_1FLAIR,軸位T_1FLAIR、T_2WI及T_2FLAIR,冠狀位3D CUBE T_2WI、T_2FLAIR及3D BRAVO。掃描后3D CUBE圖像經(jīng)多層面重組技術(shù)合成多方位及多種厚度的2D影像,與常規(guī)MRI對病變的顯示能力進行比較,采用SPSS 17.0進行處理分析,經(jīng)卡方檢驗,P0.05認為差異有統(tǒng)計學意義。結(jié)果 30例患者中,3D CUBE重組圖像發(fā)現(xiàn)腦內(nèi)陽性病灶29個,而常規(guī)MRI發(fā)現(xiàn)陽性病灶15個,3D CUBE技術(shù)比常規(guī)MRI的敏感度高46.7%(P0.05),差異有顯著意義。結(jié)論由于顳葉相鄰結(jié)構(gòu)復(fù)雜,當致癇灶病理改變不明顯時,應(yīng)運用3D CUBE掃描技術(shù),然后采取多角度及薄層高分辨2D重組圖像,有利于顱內(nèi)顳葉病變的檢出。
[Abstract]:Objective to evaluate the clinical value of magnetic resonance isotropic volume scanning (CUBEB) imaging in the detection of brain lesions in temporal lobe epilepsy (TLEs). Methods from June 2015 to September 2016, 30 patients with epilepsy, aged 765 years, with an average age of 31.7 years, were collected. All MRI scans were performed with sagittal T _ 1FLAIRI, T _ 1F _ LAIRI T _ 2WI and T _ 2F _ LAIRI, 3D CUBE T _ 2W _ Iar _ T _ 2FLAIR and 3D Bravo. After scanning 3D CUBE images were synthesized into multi-directional and multi-thickness 2D images by multi-level recombination technique, and compared with conventional MRI, SPSS 17.0 was used to process and analyze the lesions. The chi-square test showed that the difference was statistically significant. Results among the 30 patients, 29 lesions were found in brain by 3D CUBE reconstruction, while 15 lesions were detected by conventional MRI. The sensitivity of 3D CUBE was 46.7% higher than that of conventional MRI (P 0.05). Conclusion because of the complex adjacent structure of temporal lobe, 3D CUBE scanning technique should be used when the pathological changes of epileptogenic foci are not obvious, and then multi-angle and thin-layer high-resolution 2D recombination images can be used to detect intracranial temporal lobe lesions.
