【摘要】：目的： 通過對(duì)鏡像療法治療中老年腦卒中偏癱患者上肢功能的療效分析，找到改善中老年腦卒中偏癱患者上肢功能的有效治療手段。 方法： 將60例住院治療的中老年腦卒中患者隨機(jī)分為A、B兩組，各組30例患者。A組作為對(duì)照組用透明塑料板代替鏡子訓(xùn)練、B組作為實(shí)驗(yàn)組利用鏡像療法訓(xùn)練，兩組均給予常規(guī)康復(fù)訓(xùn)練。分別于治療前及4周治療后，，采用Fugl-Meyer（FMA）上肢運(yùn)動(dòng)評(píng)價(jià)、上肢運(yùn)動(dòng)研究測(cè)試（ARAT）、運(yùn)動(dòng)功能評(píng)估量表（MAS）對(duì)上肢運(yùn)動(dòng)功能進(jìn)行評(píng)價(jià)，改良Barthel指數(shù)（MBI）對(duì)患者的日常生活活動(dòng)能力進(jìn)行評(píng)估。 結(jié)果： 在治療前，兩組患者的性別、年齡（45歲患者）、病程、腦卒中類型、偏癱側(cè)別等比較，差異無統(tǒng)計(jì)學(xué)意義（p0.05）。 1．鏡像治療組與對(duì)照組治療前后偏癱側(cè)上肢運(yùn)動(dòng)功能的比較 治療4周后，2組FMA、ARAT、MAS評(píng)分均較治療前提高（P0.05)，且B組更高于A組（P0.05)。 2.鏡像治療組與對(duì)照組治療前后日常生活活動(dòng)能力的比較 治療4周后，2組MBI評(píng)分均較治療前提高（P0.05)，且B組更高于A組（P0.05)。 3.對(duì)鏡像治療組患者進(jìn)行表面肌電圖采集結(jié)果 鏡像治療組肌電圖和收集到的肌電圖數(shù)值均較治療前提高。 結(jié)論： 在常規(guī)康復(fù)治療基礎(chǔ)上，聯(lián)合鏡像療法的治療手段能夠有效地提高中老年偏癱患者上肢的運(yùn)動(dòng)功能、縮短療程，對(duì)其日常生活活動(dòng)能力也有大幅度的提高。此聯(lián)合療法療效顯著，使患者能夠早日回歸家庭，回歸社會(huì)。
[Abstract]:Objective:. By analyzing the curative effect of mirror image therapy on upper limb function of middle and old stroke patients with hemiplegia, an effective treatment method was found to improve upper limb function of middle and old stroke patients with hemiplegia. Methods:. Sixty middle-aged and aged stroke patients who were hospitalized were randomly divided into two groups: group A (n = 30) and group A (n = 30) as control group. Group B was treated with transparent plastic plate instead of mirror training and group B was trained by mirror therapy. The two groups were given routine rehabilitation training before and after 4 weeks of treatment. The upper limb motor function was evaluated by Fugl-Meyerman FMA, the upper limb motor research test and motor function assessment scale (MASS). Modified Barthel index was used to evaluate the activity of daily living (ADL) of patients. Results:. Before treatment, there was no significant difference in sex, age and age of 45 years old patients, course of disease, stroke type, hemiplegic side and so on. There was no significant difference between the two groups (p 0.05). 1. Comparison of the motor function of the hemiplegic upper limb between the mirrored treatment group and the control group before and after treatment. After 4 weeks of treatment, the FMA-ARATMA MAS scores in both groups were higher than those before treatment (P 0.05), and the scores in group B were higher than those in group A (P 0.05). 2. Comparison of activities of Daily living before and after treatment between the mirrored treatment group and the control group. After 4 weeks of treatment, the MBI scores in both groups were higher than those before treatment, and the scores in group B were higher than those in group A (P 0.05). 3.The surface electromyography of the patients in the mirror treatment group was collected. The electromyography (EMG) and the electromyogram (EMG) collected in the mirrored treatment group were higher than those before treatment. Conclusion:. On the basis of conventional rehabilitation therapy, combined with mirror image therapy can effectively improve the upper limb motor function of middle-aged and old patients with hemiplegia and shorten the course of treatment. The effect of this combination therapy is remarkable, which enables the patients to return to their families and society as soon as possible.
