頸總動脈血流動力學(xué)參數(shù)判斷頸內(nèi)動脈狹窄程度的價值

【摘要】:目的探討頸總動脈血流動力學(xué)參數(shù)判斷頸內(nèi)動脈狹窄程度的價值。方法選擇單側(cè)頸內(nèi)動脈中度及以上狹窄或閉塞、對側(cè)同名動脈無狹窄或輕度狹窄者255例,根據(jù)頸內(nèi)動脈狹窄程度分為頸內(nèi)動脈中度狹窄98例(中度組)、重度狹窄89例(重度組)、閉塞68例(閉塞組)。分別測量各組頸總動脈收縮期血流峰值(PSV)、舒張末期血流速度(EDV)、阻力指數(shù)(RI)、血管內(nèi)徑(D)、血流量(BFV),并分析各指標(biāo)與頸內(nèi)動脈狹窄程度的關(guān)系。結(jié)果重度組和閉塞組患側(cè)頸總動脈PSV、EDV、BFV明顯低于同組健側(cè)頸總動脈,RI明顯高于同組健側(cè)頸總動脈(P均0.05)。重度組和閉塞組患側(cè)頸總動脈PSV、EDV、BFV明顯低于中度組患側(cè)頸總動脈,RI明顯高于中度組患側(cè)頸總動脈(P均0.05)。相關(guān)分析顯示,頸內(nèi)動脈狹窄程度與頸總動脈EDV、BFV呈負(fù)相關(guān)關(guān)系(r分別為-0.893、-0.723,P均0.01),與頸總動脈RI呈正相關(guān)關(guān)系(r=0.853,P0.05),與頸總動脈PSV、D無明顯相關(guān)性(r分別為0.536、0.324,P均0.05)。結(jié)論頸總動脈EDV、BFV可用于評價頸內(nèi)動脈狹窄程度,尤其是頸內(nèi)動脈閉塞。
[Abstract]:Objective to evaluate the value of common carotid artery hemodynamic parameters in judging the degree of internal carotid artery stenosis. Methods 255 cases of unilateral internal carotid artery stenosis or occlusion without stenosis or slight stenosis of the contralateral artery with the same name were selected. According to the degree of internal carotid artery stenosis, 98 cases of internal carotid artery stenosis (moderate group), 89 cases of severe stenosis (severe group) and 68 cases of occlusion (occlusion group) were divided into two groups. The blood flow velocity, resistance index (RI), blood flow volume (BFV) and blood flow volume (BFV) were analyzed. Results the BFV of common carotid artery in severe group and occlusion group was significantly lower than that in the control group, and the RI of common carotid artery in normal side was significantly higher than that in control group. In the same group, the normal common carotid artery (P < 0.05), the common carotid artery (PSV) EDV (BFV) in the severe group and the occlusion group (P < 0.05) were significantly lower than those in the moderate group (P < 0.05). There was a negative correlation between the degree of stenosis of internal carotid artery and EDVV BFV of common carotid artery (r = -0.893t -0.723), a positive correlation with RI of common carotid artery (r = 0.853N, P 0.05), and no significant correlation with common carotid artery (PSV) D, respectively (r = 0.5360.3244P = 0.050.Conclusion EDV BFV of common carotid artery can be used to evaluate common carotid artery. Degree of stenosis of internal carotid artery, Especially internal carotid artery occlusion.
