【摘要】：報(bào)告1例皮脂腺痣并發(fā)基底細(xì)胞癌及管狀乳頭狀大汗腺腺瘤�；颊吣�,43歲。因右側(cè)頭部斑塊43年就診。皮膚科檢查:右側(cè)頭部見淡黃色丘疹、結(jié)節(jié)及疣狀斑塊,質(zhì)軟,邊界清楚,局部見數(shù)個(gè)大小不等黑色結(jié)節(jié)。皮損組織病理檢查示(1)切片右側(cè):表皮角化過度伴角化不全,真皮淺、中層可見大小不等的腫瘤細(xì)胞團(tuán)塊,部分和表皮相連,腫瘤細(xì)胞團(tuán)塊由基底樣細(xì)胞組成,腫瘤團(tuán)塊周邊細(xì)胞呈柵欄狀排列并可見收縮間隙;(2)切片左側(cè):表皮角化過度、棘層肥厚,乳頭瘤樣增生,真皮淺、中層可見增生的皮脂腺及以淋巴細(xì)胞為主的炎性細(xì)胞浸潤,真皮深層見大汗腺分布,形成較多管腔樣結(jié)構(gòu),管腔內(nèi)見大汗腺呈乳頭狀突入管腔。
[Abstract]:A case of sebaceous nevus complicated with basal cell carcinoma and tubular papillary sweat gland adenoma was reported. The patient was 43 years old. He was treated for 43 years because of plaque on the right side of the head. Dermatology examination: right head light yellow papules, nodules and verrucous plaques, soft, clear border, several local black nodules of varying sizes. Histopathological examination of the lesions showed that (1) on the right side of the section: the keratosis of the epidermis was hyperkeratosis with hypokeratosis, the dermis was shallow, and the tumor cell mass of different sizes was seen in the middle layer, some of which were connected with the epidermis, and the tumor cell mass was composed of basal cells. The peripheral cells of the tumor mass were arranged in a palisade shape and the contraction space could be seen. (2) on the left side of the section: hyperkeratosis of the epidermis, hypertrophy of the spinous layer, papillomatous hyperplasia, superficial dermis, hyperplastic sebaceous glands and inflammatory cell infiltration mainly with lymphocytes were seen in the middle layer of the dermis, and the hypersweat glands were found in the deep layer of the dermis. Many lumen-like structures were formed in which the sweat glands appeared papillary protrusions into the lumen.
