RECK和MMP-2在皮膚鱗狀細胞癌中的表達及臨床意義

【摘要】:目的:觀察基質(zhì)金屬蛋白酶抑制因子(RECK)和基質(zhì)金屬蛋白酶-2(MMP-2)在皮膚鱗狀細胞癌(CSCC)、鮑溫氏病、非典型增生組織中的表達,并探討其與腫瘤發(fā)生、浸潤轉(zhuǎn)移的關(guān)系。 方法:采用免疫組化PV6000法檢測55例(?)JCSCC組織(其中高分化27例、中分化18例、低分化10例;伴淋巴轉(zhuǎn)移10例,遠處轉(zhuǎn)移2例)、13例鮑溫氏病、24例不典型增生組織以及20例正常皮膚組織中的RECK和MMP-2蛋白表達,進而比較鱗狀細胞癌在不同分化組、轉(zhuǎn)移組、年齡及性別組中的表達情況,并將之結(jié)果進行統(tǒng)計學(xué)分析。 結(jié)果: 1鏡下觀察RECK的陽性表達主要位于正常鱗狀上皮細胞及腫瘤細胞的細胞質(zhì)中,呈淺黃色至棕黃色顆粒,部分胞核染色。MMP-2的陽性染色多定位于癌組織細胞質(zhì),呈淺黃色至棕黃色顆粒。 2 RECK在正常皮膚組織(CE)、非典型增生(AH)、鮑溫氏病(BD)、皮膚鱗狀細胞癌(CSCC)四種組織中的表達RECK在正常皮膚組織、非典型增生、鮑溫氏病、皮膚鱗狀細胞癌中的表達率分別為85.0%、54.2%、46.2%、43.6%,比較差異有統(tǒng)計學(xué)意義(P0.05)。 3 MMP-2蛋白正常皮膚組織(CE)、非典型增生(AH)、鮑溫氏病(BD)、皮膚鱗狀細胞癌(CSCC)四種組織中的表達MMP-2蛋白在正常皮膚組織、非典型增生、鮑溫氏病、皮膚鱗狀細胞癌中的表達率分別為25.0%、50.0%、53.8%、63.6%,比較差異有統(tǒng)計學(xué)意義(P0.05)。 4 RECK及MMP-2蛋白表達與皮膚鱗狀細胞癌(CSCC)的組織學(xué)分級的關(guān)系根據(jù)組織學(xué)分級,皮膚鱗狀細胞癌組織分為高中低分化三組。RECK陽性表達率隨著腫瘤分化程度的降低而降低;而MMP-2陽性表達率隨著腫瘤分化程度的降低而增高。差異均有統(tǒng)計學(xué)意義(P0.05)。 5 RECK及MMP-2蛋白與皮膚鱗狀細胞癌(CSCC)淋巴轉(zhuǎn)移和遠處轉(zhuǎn)移的關(guān)系無淋巴轉(zhuǎn)移組中RECK陽性表達率明顯高于淋巴轉(zhuǎn)移組,比較差異有統(tǒng)計學(xué)意義(P0.01)。MMP-2在有淋巴轉(zhuǎn)移的CSCC組中陽性表達率明顯高于無淋巴轉(zhuǎn)移組,兩者表達差異有統(tǒng)計學(xué)意義(P0.01)。RECK (?)口MMP-2與鱗狀細胞癌是否存在遠處轉(zhuǎn)移無關(guān),表達無統(tǒng)計學(xué)意義(P0.05)。 6 RECK及MMP-2蛋白與皮膚鱗狀細胞癌(CSCC)年齡和性別的關(guān)系RECK、MMP-2蛋白表達與皮膚鱗狀細胞癌患者的年齡及性別無關(guān),差異無統(tǒng)計學(xué)意義(P0.05)。 7 RECK與MMP-2在皮膚鱗狀細胞癌(CSCC)中表達的關(guān)系RECK及MMP-2在皮膚鱗狀細胞癌中的表達呈負相關(guān)。(r=-0.402 P0.01)。 結(jié)論: 1研究顯示RECK在正常皮膚、非典型增生、鮑溫氏病及鱗狀細胞癌中均有表達,但表達水平依次減少,而MMP-2的表達水平依次增高,提示RECK的表達下調(diào)或缺失及MMP-2的過度表達在鱗狀細胞癌的發(fā)生發(fā)展過程中可能起著重要作用。 2隨著腫瘤組織學(xué)分化程度降低,RECK (?)日性率顯著降低,MMP-2表達升高,提示RECK作為抑癌因子可能通過誘導(dǎo)細胞分化從而抑制鱗狀細胞癌的發(fā)生,而MMP-2的上調(diào)表達則抑制了細胞分化過程,從而導(dǎo)致腫瘤惡性程度增高。兩者可能為判斷惡性程度提供依據(jù)。 3本研究發(fā)現(xiàn)RECK、MMP-2蛋白表達與淋巴結(jié)轉(zhuǎn)移的發(fā)生顯著相關(guān),提示RECK表達降低,而MMP-2蛋白表達上調(diào)的腫瘤具有更強的侵蝕和轉(zhuǎn)移能力。 4在鱗狀細胞癌組織中RECK的低表達與MMP-2的表達升高有關(guān)。在皮膚鱗狀細胞癌的發(fā)生發(fā)展過程中,兩者呈顯著的負相關(guān)。提示鱗狀細胞癌組織中RECK表達的缺失導(dǎo)致了對MMP-2的抑制作用降低,而使MMP-2表達上調(diào),進而促進了腫瘤細胞的侵襲和轉(zhuǎn)移。
