【摘要】：目的探討Mohs顯微描記手術治療頭面部基底細胞癌、鱗狀細胞癌及鮑溫病等皮膚惡性腫瘤的療效和預后,分析頭面部皮膚惡性腫瘤的臨床特點。方法該院收治的未見轉移的面部皮膚惡性腫瘤患者411例,分為Mohs手術組256例,傳統(tǒng)術中冷凍手術組130例,經(jīng)驗切除組25例,并對切取腫瘤創(chuàng)面采用鄰近皮瓣、肌皮瓣及游離皮片或直接縫合方式修復。隨訪3~5年。結果 Mohs手術組248例手術切口Ⅰ期愈合,皮瓣或皮片全部成活,8例Ⅱ期愈合,2例復發(fā)(0.8%)。傳統(tǒng)術中冷凍手術組125例手術切口Ⅰ期愈合,5例Ⅱ期愈合,7例復發(fā)(5.4%)。經(jīng)驗切除組25例均Ⅰ期愈合,4例復發(fā)(16%)。各組術后病理復檢發(fā)現(xiàn),腫瘤殘余和臨床復發(fā)率差異有統(tǒng)計學意義(P0.01)。結論 Mohs手術可以在完整切除腫瘤的同時盡可能保留正常組織減少外觀影響,是目前治療頭面部皮膚惡性腫瘤的首選方案。
[Abstract]:Objective to evaluate the efficacy and prognosis of Mohs micrography in the treatment of skin malignant tumors such as basal cell carcinoma, squamous cell carcinoma and Bowen's disease. Methods the clinical features of malignant tumors of head and face were analyzed. Methods 411 cases of facial malignant tumors without metastasis were divided into three groups: Mohs group (n = 256), conventional cryopreservation group (n = 130) and experiential resection group (n = 25). The tumor wounds were repaired with adjacent flaps, myocutaneous flaps and free skin grafts or direct suture. All the skin flaps or skin grafts survived in 8 cases with stage 鈪，
