【摘要】：目的:探討益氣補(bǔ)脾法對脾虛證2型糖尿病患者腸道菌群及脂質(zhì)代謝的影響。方法:選取2014年5月—2016年5月遼寧中醫(yī)藥大學(xué)附屬醫(yī)院內(nèi)分泌K科收治的95例脾虛證2型糖尿病患者為研究對象,按隨機(jī)數(shù)字表法將所有患者隨機(jī)分為對照組(n=47)和觀察組(n=48),選取40例本院同期體檢健康者作為健康對照,對照組患者均給予常規(guī)基礎(chǔ)治療+二甲雙胍片250 mg/次,3次/d,觀察組加用降糖丸,1袋3次/d沖服,治療前后提取糞便菌群基因組DNA,通過構(gòu)建糞便菌群的細(xì)菌通用16S rRNA基因克隆文庫分析患者腸道菌群變化,治療2個月,觀察患者治療前后中醫(yī)證候積分,血脂、血糖、胰島素水平及腸道菌群變化。結(jié)果:觀察組的證候療效總有效率明顯高于對照組(P0.05),治療后兩組中醫(yī)證候積分、血脂、血糖及胰島素指標(biāo)(空腹胰島素、胰島素抵抗指數(shù))有明顯改善,腸道菌群中擬桿菌屬、乳酸桿菌屬、雙歧桿菌屬數(shù)量升高,腸球菌屬、腸桿菌、酵母菌數(shù)量降低(P0.05),治療后中醫(yī)證候積分、血脂、血糖、胰島素指標(biāo)(空腹胰島素、胰島素抵抗指數(shù))及腸道菌群數(shù)量觀察組與對照組比較差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:益氣補(bǔ)脾法可有效改善脾虛證2型糖尿病患者腸道菌群失調(diào),血糖、血脂代謝紊亂,有助于改善患者預(yù)后情況。
[Abstract]:Objective: to study the effect of invigorating qi and invigorating spleen on intestinal flora and lipid metabolism in type 2 diabetes mellitus with spleen deficiency syndrome. Methods: from May 2014 to May 2016, 95 patients with type 2 diabetes mellitus with spleen deficiency syndrome were selected from Department of Endocrine K, affiliated Hospital of Liaoning University of traditional Chinese Medicine, as the study objects. All the patients were randomly divided into two groups: control group (n = 47) and observation group (n = 48). 40 healthy persons in the same period were selected as healthy control group. All patients in control group were treated with metformin tablets for 250 mg/. Three times a day, the observation group was treated with Jiangtang pills, a bag and three times a day. The genomic DNA, of fecal flora was extracted before and after treatment. The bacterial 16s rRNA gene clone library of fecal flora was constructed to analyze the changes of intestinal flora of patients, and the patients were treated for 2 months. To observe the changes of TCM syndrome score, blood lipid, blood glucose, insulin level and intestinal flora before and after treatment. Results: the total effective rate of syndrome in the observation group was significantly higher than that in the control group (P0.05). After treatment, the TCM syndromes integral, blood lipid, blood glucose and insulin index (fasting insulin, insulin resistance index) were significantly improved in the two groups. Intestinal flora of Bacteroides, Lactobacillus, Bifidobacterium increased, Enterococcus, Enterobacter, yeast decreased (P0.05). After treatment, TCM syndrome score, blood lipid, blood glucose, insulin index (fasting insulin), Insulin resistance index (IR) and the number of intestinal flora in the observation group compared with the control group were significantly different (P0.05). Conclusion: the method of invigorating qi and invigorating spleen can effectively improve the intestinal flora disorder, blood sugar and lipid metabolism disorder in type 2 diabetes mellitus with spleen deficiency syndrome, and it is helpful to improve the prognosis of the patients.
