【摘要】：目的探討DN患者血清中VEGF表達水平的變化及其與尿微量白蛋白的關系,為DN蛋白尿的治療提供新的治療方向。方法54例糖尿病患者,依據(jù)24h尿微量白蛋白定量,將其分為三組:正常蛋白尿組、微量蛋白尿組、大量蛋白尿組,同期20例門診健康體檢者正常對照組。采用雙抗體夾心酶聯(lián)免疫吸附法(ELISE)檢測血清中VEGF的水平,同時收集空腹血糖、血肌酐、尿素氮、糖化蛋白、收縮壓及舒張壓等臨床指標。結果1.與對照組相比,微量及大量蛋白尿組血清中VEGF水平明顯升高(P0.05),而正常蛋白尿組僅表現(xiàn)出上升趨勢(P=0.630.05);2.與正常蛋白尿組比較,大量蛋白尿組血清中VEGF的表達水平明顯升高(P0.05),而微量蛋白尿組則表現(xiàn)為上升趨勢(P=0.0770.05);3.血清中VEGF的水平與空腹血糖(r=0.629,P0.01)、尿微量白蛋白(r=0.377,P0.01)呈顯著正相關關系,與病程、血肌酐、尿素氮、尿酸、糖化蛋白、C-反應蛋白、收縮壓及舒張壓無相關關系。結論糖尿病腎病患者血清中VEGF的表達異常升高,并且與尿微量白蛋白呈正相關關系;抑制VEGF的過度表達,可能是一種防治蛋白尿形成的重要手段。
[Abstract]:Objective to investigate the changes of serum VEGF expression in patients with DN and its relationship with urinary microalbumin in order to provide a new therapeutic direction for the treatment of DN proteinuria. Methods 54 patients with diabetes mellitus were divided into three groups according to 24 hours urinary microalbuminuria: normal albuminuria group, microalbuminuria group, macroalbuminuria group, and normal control group of 20 healthy outpatients at the same time. Double antibody sandwich enzyme linked immunosorbent assay (ELISE) was used to detect the level of serum VEGF. Fasting blood glucose, serum creatinine, urea nitrogen, glycosylated protein, systolic blood pressure and diastolic blood pressure were collected. Result 1. Compared with the control group, the serum VEGF level in the microalbuminuria group and the massive proteinuria group was significantly higher (P0.05), while the normal proteinuria group only showed an upward trend (P0. 630. 05); 2. Compared with normal proteinuria group, the expression of VEGF in serum of large proteinuria group was significantly higher than that of microalbuminuria group (P0.05), while that of microalbuminuria group was increasing (P0. 077. 05); 3. The level of VEGF in serum was positively correlated with fasting blood glucose (r = 0.629, P 0.01) and urinary microalbumin (r = 0.377N, P 0.01), and with the course of disease, serum creatinine, urea nitrogen, uric acid, glycosylated protein, C-reactive protein, serum creatinine, urea nitrogen, uric acid, glycosylated protein, C-reactive protein. There was no correlation between systolic blood pressure and diastolic blood pressure. Conclusion the expression of VEGF in serum of patients with diabetic nephropathy is abnormally elevated and positively correlated with urinary albumin, and inhibiting the overexpression of VEGF may be an important means to prevent the formation of proteinuria.
