【摘要】：目的:觀察哮喘-COPD重疊綜合征(Asthma-COPD overlap syndrome,ACOS)機械通氣患者肺泡灌洗液(BALF)中肺表面活性物質相關蛋白D(SP-D)及白介素-8(IL-8)的水平,探討其對ACOS機械通氣患者病情發(fā)展影響。方法:38例ACOS機械通氣患者根據入院第1天和第5天的APACHEⅡ評分變化,分為病情加重組(18例),及病情緩解組(20例),收集機械通氣1、5 d的肺泡灌洗液,測定BALF中SP-D、IL-8濃度,觀察不同預后患者BALF中SP-D、IL-8水平的差異。結果:病情緩解組BALF中SP-D含量逐漸上升,機械通氣5 d較1 d明顯升高(P0.05);與病情緩解組比較,病情加重組機械通氣1 d、5 d BALF中SP-D含量均明顯降低(P0.05)。病情緩解組BALF中IL-8含量逐漸下降,病情加重組BALF中IL-8含量逐漸上升(P0.05)。統(tǒng)計分析結果提示BALF中IL-8含量與APACHE II評分呈負相關(r為-0.44,P0.05);統(tǒng)計分析結果提示BALF中SP-D濃度與APACHE II評分也呈負相關(r為-0.32,P0.05)。結論:SP-D、IL-8在ACOS患者的發(fā)生發(fā)展中起重要的作用,BALF中SP-D、IL-8可作為ACOS機械通氣患者病情監(jiān)測和預后的參考指標。
