糖尿病教育對糖尿病前期患者轉歸影響的研究

【摘要】:目的:研究糖尿病教育對糖尿病前期患者體質(zhì)指數(shù)、腰圍、血壓等生理參數(shù)指標和空腹血糖、口服葡萄糖耐量試驗2小時血糖、糖化血紅蛋白等血糖指標及轉歸的影響;方法:將在社區(qū)免費體檢篩查出的120例糖尿病前期患者,根據(jù)其自身是否愿意接受糖尿病教育分為教育組和對照組。教育組開展為期1年的糖尿病教育,對照組可從全科醫(yī)生獲得簡單口頭教育,或是從電視、網(wǎng)絡及其它有關宣傳資料中了解糖尿病一般知識,不進行專門的糖尿病教育。觀察隨訪1年后對兩組人群體質(zhì)指數(shù)、腰圍、血壓等生理參數(shù)指標和空腹血糖、口服葡萄糖耐量試驗2小時血糖、糖化血紅蛋白等血糖指標及轉歸情況進行評價;結果:兩組基線時一般資料、生理參數(shù)及血糖水平均無差異。觀察隨訪1年后,教育組體質(zhì)指數(shù)、腰圍、血壓等生理參數(shù)和口服葡萄糖耐量試驗2小時血糖、糖化血紅蛋白等血糖指標較對照組有明顯降低,差異有統(tǒng)計學意義(P0.05),教育組空腹血糖在隨訪后與對照組比較有所改善,但差異不具有統(tǒng)計學意義(P0.05)。糖尿病前期患者轉化率方面:教育組糖耐量正常逆轉率明顯高于對照組(P0.01),教育組糖尿病轉化率明顯低于對照組(P0.05);結論:糖尿病教育可改善糖尿病前期患者多項代謝指標,降低糖尿病發(fā)病風險并增加糖耐量正常逆轉率。糖尿病教育可有效控制糖尿病前期患者病情防治糖尿病,適合社區(qū)開展,便于推廣,為社區(qū)糖尿病防治提供科學依據(jù)。
[Abstract]:Objective: to study the effects of diabetes education on body mass index (BMI), waist circumference, blood pressure and fasting blood glucose (FBG), blood glucose and glycosylated hemoglobin (HbA1c) in patients with prediabetes mellitus in 2 hours oral glucose tolerance test (OGTT). Methods: 120 prediabetic patients who were screened by free physical examination in the community were divided into education group and control group according to their willingness to receive diabetes education. The education group carries on the diabetes education for one year, the control group can obtain simple oral education from the general practitioner, or learn the general knowledge of diabetes from TV, network and other related propaganda materials, do not carry on the special diabetes education. Body mass index (BMI), waist circumference, blood pressure and fasting blood glucose (FBG), blood glucose (GTT), glycosylated hemoglobin (HbHb) and other blood glucose indexes were observed and evaluated after 1 year follow-up. Results: there was no difference in general data, physiological parameters and blood glucose level between the two groups. The body mass index, waist circumference, blood pressure and other physiological parameters in the education group were significantly lower than those in the control group after 1 year follow-up (P0.05), and the blood glucose and glycosylated hemoglobin were significantly lower in the education group than in the control group (P0.05). The fasting blood glucose in the education group was improved after follow-up compared with the control group, but the difference was not statistically significant (P0.05). The conversion rate of pre-diabetic patients: the normal reversal rate of glucose tolerance in the education group was significantly higher than that in the control group (P0.01), and the conversion rate of diabetes in the education group was significantly lower than that in the control group (P0.05). Conclusion: diabetes education can improve many metabolic indexes, reduce the risk of diabetes and increase the normal reversal rate of glucose tolerance. Diabetes education can effectively control the condition of pre-diabetic patients to prevent and treat diabetes, suitable for community development, easy to promote, and provide scientific basis for community diabetes prevention and treatment.
