【摘要】：目的評(píng)價(jià)加味四妙丸治療急性痛風(fēng)性關(guān)節(jié)炎(AGA)的臨床療效。方法選取AGA患者60例,隨機(jī)分為西藥組和中西藥組各30例。西藥組予雙氯芬酸鈉緩釋片口服,每次75 mg,每日1次;碳酸氫鈉片口服,每次1000 mg,每日3次。中西藥組在西藥組基礎(chǔ)上聯(lián)合加味四妙丸。兩組均治療3天,治療期間患者如因關(guān)節(jié)疼痛加重經(jīng)評(píng)定改用糖皮質(zhì)激素治療。分別于治療前后檢測(cè)患者血白細(xì)胞(WBC)、C反應(yīng)蛋白(CRP)、丙氨酸氨基轉(zhuǎn)移酶(ALT)、門冬氨酸氨基轉(zhuǎn)移酶(AST)、血尿酸(UA)、血肌酐(Cr)水平,并進(jìn)行關(guān)節(jié)疼痛評(píng)分和關(guān)節(jié)紅腫評(píng)分。比較兩組患者改用糖皮質(zhì)激素的比例。結(jié)果中西藥組改用糖皮質(zhì)激素治療2例,明顯低于西藥組的9例(P0.05);中西藥組治療后WBC、CRP、UA水平及關(guān)節(jié)疼痛評(píng)分、關(guān)節(jié)紅腫評(píng)分均較治療前明顯降低(P0.01),且CRP、UA水平及關(guān)節(jié)疼痛評(píng)分、關(guān)節(jié)紅腫評(píng)分均低于西藥組(P0.05)。結(jié)論加味四妙丸治療AGA可抑制炎癥反應(yīng),緩解關(guān)節(jié)腫痛癥狀,減少糖皮質(zhì)激素使用。
[Abstract]:Objective to evaluate the clinical effect of Jiawei Simiao pill on acute gouty arthritis (AGA). Methods 60 patients with AGA were randomly divided into western medicine group (30 cases) and traditional Chinese medicine group (30 cases). The western medicine group was treated with diclofenac sodium sustained release tablet once a day for 75 mg, and sodium bicarbonate tablet 3 times a day for 1000 mg,. The Chinese and western medicine group combined with modified Simiao pills on the basis of western medicine group. Both groups were treated with glucocorticoid for 3 days. Before and after treatment, the levels of serum creatinine (Cr) (Cr) in patients with leukocyte (WBC), C reactive protein (CRP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), serum creatinine (Cr), joint pain score and joint redness were measured. The proportion of patients using glucocorticoid was compared between the two groups. Results two cases were treated with glucocorticoid in the Chinese and western medicine group, which were significantly lower than those in the western medicine group (P 0.05), the WBC,CRP,UA level, joint pain score and joint redness score were significantly lower in the Chinese and western medicine group than before treatment (P0.01), and the CRP,UA level and joint pain score were significantly lower in the Chinese and western medicine group than before treatment (P0.01). The score of joint redness and swelling was lower than that of western medicine group (P0.05). Conclusion Jiawei Simiao Pill can inhibit inflammation, relieve joint swelling and pain and reduce glucocorticoid use.
