成年男性高紅癥對HbALc的影響研究

【摘要】:目的:相對低海拔及平原地區(qū),高原地區(qū)由于氧分壓低,Hb水平相對較高,通過比較不同Hb水平下,高原地區(qū)居民HbA1c及其影響因素,分析高紅癥對HbA1c的影響,為高原地區(qū)HbA1c的切點提供參考依據。方法:本研究收集2013年10月至2015年12月期間在青海大學附屬醫(yī)院就診的172例非糖尿病成年男性資料,年齡在20-75歲之間,均為世居或移居(≥3月)的成年漢族男性,居住海拔范圍2700-3000m。以Hb水平不同分為三組,正常Hb組56例、高Hb組61例、高紅組55例,比較三個組的HbA1c值,分析高紅癥對HbA1c的影響。結果:1、不同Hb組一般臨床資料比較,對于年齡、RBC、HCT水平,三組比較均有顯著差異(P0.001);對于BMI、FBG、HbA1c,三組比較均無統(tǒng)計學差異(P0.05)。2、高原地區(qū)成年男性HbA1c的主要影響因素是FBG,經線性相關分析,HbA1c與FBG呈正相關,差異有統(tǒng)計學意義(p0.05);3、正常Hb組HbA1c的影響因素是年齡、Hb、FBG,經線性相關分析,HbA1c與Hb負相關,差異有統(tǒng)計學意義(p0.05)。4、高Hb組HbA1c的影響因素是年齡,高紅組HbA1c的影響因素是FBG,差異均有統(tǒng)計學意義(p0.05)。結論:1、本研究顯示成年男性HbA1c的影響因素除FBG、年齡外,還受Hb水平影響,當120g/L≤Hb160g/L時,HbA1c隨Hb水平升高有下降趨勢,當Hb≥160g/L時,Hb對HbA1c無影響。2、本研究顯示成年男性高紅癥對HbA1c無影響。
