【摘要】：目的了解影響浦東新區(qū)低中檔暗娼人群(FSW)安全套使用的因素,及艾滋病病毒(HIV)、梅毒的感染情況,為調(diào)整該人群的干預(yù)方案提供依據(jù)。方法將浦東新區(qū)按照合并前的兩個(gè)行政區(qū)劃分為南片(原南匯區(qū))和北片(原浦東新區(qū))。用方便抽樣,分別在南、北片低中檔高危場(chǎng)所內(nèi)招募FSW,參加一對(duì)一、面對(duì)面的問(wèn)卷調(diào)查(包括人口學(xué)特征、行為、干預(yù)等信息)和HIV及梅毒螺旋體的血清學(xué)檢測(cè)。結(jié)果南、北片各有400名和406名FSW完成問(wèn)卷調(diào)查及血清學(xué)檢測(cè)。北片F(xiàn)SW的平均年齡為(31.8±7.5)歲,南片為(34.9±7.9)歲;北片F(xiàn)SW的文化程度在初中及以上者占80.0%,南片為61.3%;北片未婚者比例為27.6%,南片為15.0%。FSW來(lái)自低檔場(chǎng)所的比例,南片為70.8%,北片為55.9%。此次調(diào)查中,有1名FSW為HIV抗體陽(yáng)性,24名FSW(南、北片分別為18人和6人)感染梅毒。南、北片F(xiàn)SW近一個(gè)月商業(yè)性行為堅(jiān)持用套的比例分別是55.3%和61.1%。在調(diào)查前30天商業(yè)性行中,北片F(xiàn)SW堅(jiān)持使用安全套的可能性是南片F(xiàn)SW的2.783倍[95%可信區(qū)間(CI):1.696~4.566];接受外展干預(yù)次數(shù)較多的FSW更可能堅(jiān)持用安全套[比值比(OR)=2.097,95%CI:1.450~3.034];且接受干預(yù)平均時(shí)間在5分鐘以上的FSW,更可能在商業(yè)行中堅(jiān)持使用安全套(OR=1.351,95%CI:1.137~2.658)。結(jié)論浦東新區(qū)中低檔FSW的HIV、梅毒呈低流行,但安全套使用比例仍有待提高;片區(qū)和干預(yù)是影響FSW安全套使用的重要因素,FSW的社區(qū)干預(yù)需兼顧社區(qū)、片區(qū)特點(diǎn)。
[Abstract]:Objective to investigate the factors affecting (FSW) condom use and HIV / AIDS (HIV), syphilis infection among low and middle class prostitutes in Pudong New area, and to provide evidence for adjusting the intervention scheme. Methods Pudong New area was divided into two administrative regions: Nanping (former Nanhui) and Beiping (former Pudong New area) according to the two administrative districts before the merger. Using convenience sampling, FSWs were recruited in low-middle risk sites in the north and south, and participated in one-to-one, face-to-face questionnaire survey (including demographic characteristics, behavior, intervention information), serological detection of HIV and Treponema pallidum (Treponema pallidum). Results there were 400 and 406 FSW patients in the south and north respectively completed the questionnaire and serological examination. The average age of FSW was (31.8 鹵7.5) years and (34.9 鹵7.9) years, the educational level of FSW was 80.010 in junior middle school and above, 61.3 in south, 27.6in north, 70.88in south and 55.9in north. In this survey, 1 FSW was HIV antibody positive and 24 FSW (18 in the south and 6 in the north) were infected with syphilis. In the last month, 55.3% and 61.1% of the FSW in the south and the north respectively insisted on using the bushing. 30 days before the survey, North FSW was 2.783 times more likely to insist on condom use than Southern FSW [95% confidence interval (CI): 1.696 4.566]; FSW with more outreaching intervention was more likely to insist on condom use than (OR) 2.09795% CI: 1.450,3.034; and FSWs with average intervention time of more than 5 minutes were more likely to adhere to condom use. They are more likely to stick to condoms in business (ORX 1.351C 95 CI: 1.137U 2.658). Conclusion there is a low prevalence of syphilis in middle and low grade FSW in Pudong New area, but the proportion of condom use still needs to be improved.
