【摘要】：目的:探討Cys-C在痛風(fēng)性關(guān)節(jié)炎患者中的水平及臨床意義。方法:納入152例痛風(fēng)性關(guān)節(jié)炎患者作為痛風(fēng)組,入選57例年齡、性別相匹配的健康體檢者作為對照組,收集兩組人員包括Cys-C在內(nèi)的生化指標(biāo)及其它臨床數(shù)據(jù),分別依據(jù)不同年齡及病程進行分組,并進行組間比較及相關(guān)性分析,比較包括Cys-C在內(nèi)的反應(yīng)腎功能受損的指標(biāo)在早期痛風(fēng)腎病中的價值。分析LVEF與Cys-C之間的關(guān)系。數(shù)據(jù)采用SPSS18.0軟件進行統(tǒng)計學(xué)分析。結(jié)果:1.痛風(fēng)組Cys-C水平(1.63±0.70)mg/l明顯高于健康對照組(0.83±0.12)mg/l,差異有統(tǒng)計學(xué)意義(P0.01);2.依據(jù)年齡將痛風(fēng)性關(guān)節(jié)炎患者分為三個亞組(青年組(45歲);中年組(45-59歲);老年組(≥60歲)),三組間Cys-C水平分別為:青年組(1.15±0.26)mg/l,中年組(1.68±0.54)mg/l,老年組(2.04±0.83)mg/l,隨著年齡段的升高,Cys-C的水平也不斷升高,組間進行SNK-q檢驗,各組間差異均具有統(tǒng)計學(xué)意義(P0.05);痛風(fēng)組青年組、中年組、老年組三組間Cys-C水平(1.15±0.26,1.68±0.54,2.04±0.83)mg/l均較對照組高(0.85±0.14,0.82±0.13,0.81±0.08)mg/l,差異均具有顯著統(tǒng)計學(xué)意義(P0.01)。3.依據(jù)痛風(fēng)性關(guān)節(jié)炎患者的病程分為兩組(病程≤5年組;病程5年組):病程5年組Cys-C水平(1.83±0.76)mg/l明顯高于病程≤5年組(1.45±0.58)mg/l,差異有顯著統(tǒng)計學(xué)意義(P0.01);4.以eGFR作為判斷腎功能受損的指標(biāo),eGFRcr判斷腎功能損害的陽性率為19.74%,低于eGFRcys(68.42%)及eGFRcr-cys(45.39%);5.LVEF異常組Cys-C水平明顯高于LVEF正常組(2.64±0.67 vs 1.32±0.32)mg/l,差異具有顯著統(tǒng)計學(xué)意義(P0.01);6.Cys-C水平與BUN、Cr、年齡、ESR等臨床指標(biāo)均具有相關(guān)性,Cys-C水平與BUA、CRP、TC、TG、HDL、LDL無相關(guān)性。結(jié)論:1.eGFRcys及eGFRcr-cys較eGFRcr更適合作為評估早期腎功能損傷發(fā)生的指標(biāo);2.Cys-C是除BUN、Cr之外另一個早期評價痛風(fēng)性關(guān)節(jié)炎患者腎功能損害的指標(biāo);3.痛風(fēng)性關(guān)節(jié)炎患者Cys-C水平升高可能提示合并心功能不全的風(fēng)險升高。
[Abstract]:Objective: to investigate the level and clinical significance of Cys-C in patients with gouty arthritis. Methods: 152 patients with gouty arthritis were selected as gout group and 57 healthy persons matched with age and sex as control group. Biochemical indexes including Cys-C and other clinical data were collected from both groups. According to the age and course of disease, the patients were divided into groups and compared with each other, and the correlation analysis was carried out to compare the value of the indexes of reactive renal function including Cys-C in early gout nephropathy. The relationship between LVEF and Cys-C was analyzed. The data were analyzed by SPSS18.0 software. The result is 1: 1.
