【摘要】：目的通過分析顱腦磁共振(MRI)彌散加權(quán)成像(DWI)、擴散張量成像(DTI)表現(xiàn)及血清學指標與認知功能量表得分,探討腎性繼發(fā)性甲狀旁腺功能亢進癥(SHPT)患者認知功能變化與MRI表現(xiàn)之間的關(guān)系。方法選取腎性SHPT住院患者49例,采用Mo CA及MMSE功能量表評價認知功能,使用顱腦MRI掃描并選用部分異向性(FA)、表觀彌散系數(shù)(ADC)評價各腦功能區(qū)白質(zhì)功能,同時記錄血清檢驗指標,統(tǒng)計并分析認知功能評分與各腦白質(zhì)功能區(qū)MRI得分及生化指標間的相關(guān)性。結(jié)果腎性SHPT患者額葉區(qū)FA及ADC得分與MMSE語言、記憶功能得分相關(guān),頂葉區(qū)FA及ADC評分與Mo CA視空間執(zhí)行能力得分相關(guān),其余各功能區(qū)MRI得分與認知功能量表得分之間無明顯相關(guān)性。腎性SHPT患者認知功能與全段甲狀旁腺素(i PTH)水平呈負相關(guān),與受教育水平呈正相關(guān),與性別、年齡、血壓、血清鈣水平等無明顯相關(guān)性。結(jié)論腎性SHPT患者i PTH及受教育水平與認知功能存在相關(guān)性;顱腦MRI的額葉及頂葉FA與ADC值對判斷腎性SHPT患者認知功能水平及功能損傷定位具有較高的診斷價值,值得臨床推廣。
[Abstract]:Objective to analyze the diffusion-weighted imaging (DWI), diffusive Zhang Liang imaging (DWI), serological indexes and cognitive function scale (MCS) of craniocerebral MRI. To investigate the relationship between cognitive function and MRI findings in patients with renal secondary hyperparathyroidism. Methods A total of 49 patients with renal SHPT were selected to evaluate cognitive function with Mo CA and MMSE functional scale. Brain MRI was used to evaluate the white matter function of various brain function areas. Brain MRI was used to evaluate the white matter function of each brain function area with partial heteromorphism FAP and apparent diffusion coefficient (ADCC). Serum test indexes were recorded at the same time. The correlation of cognitive function scores with MRI scores and biochemical indexes in white matter functional areas was analyzed. Results the scores of FA and ADC in frontal lobe area were correlated with MMSE language and memory function in renal SHPT patients, and FA and ADC scores in parietal lobe area were correlated with visual executive ability of Mo-CA. There was no significant correlation between MRI scores and cognitive function scale scores in other functional areas. The cognitive function of patients with renal SHPT was negatively correlated with the whole level of parathyroid hormone (PTH), and positively correlated with the level of education, but not with sex, age, blood pressure, serum calcium level and so on. Conclusion there is a correlation between I PTH, education level and cognitive function in patients with renal SHPT, FA and ADC values in frontal and parietal lobe of craniocerebral MRI are valuable in the diagnosis of cognitive function and location of functional injury in patients with renal SHPT, and it is worth popularizing in clinic.
