廣西柳江縣成人肥胖測量指標(biāo)與血壓、血糖、血脂的相關(guān)因素研究

【摘要】:目的:探討廣西柳江縣成人中以體重指數(shù)(BMI)、腰圍(WC)、內(nèi)臟脂肪指數(shù)(VAI)為代表的肥胖指標(biāo)與血壓、血糖、血脂的關(guān)系。為預(yù)防和控制高血壓、糖尿病、高血脂等肥胖相關(guān)慢性疾病的發(fā)生提供參考依據(jù)。方法:2013年12月至2014年1月,對柳江縣人群進(jìn)行流行病學(xué)調(diào)查681人,實際有效數(shù)值399例。記錄一般測量指標(biāo),包括身高、體重、腰圍、內(nèi)臟脂肪率、血壓,并進(jìn)一步抽取血液標(biāo)本送至實驗室查空腹血糖(FPG)、總膽固醇(TC)、甘油三酯(TG)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDL-C)。結(jié)果:①在廣西柳江縣399名受檢者中,男性較女性更易患肥胖癥、血壓異常、血脂異常。②分別依據(jù)BMI、WC、VAI數(shù)值進(jìn)行分組后比較,血壓異常(高于正常范圍)、高血糖、血脂異常檢出率(患病率)的差異有統(tǒng)計學(xué)意義,超重及肥胖組中上述病癥的患病率高于非超重或肥胖組。③男性中,VAI與血壓、TG呈正相關(guān),與HDL-C呈負(fù)性相關(guān),具有統(tǒng)計學(xué)意義(p0.05);女性中,VAI與血壓、血脂均存在明顯相關(guān)關(guān)系(p0.05):VAI與MAP(平均動脈壓)、TG、TC、LDL-C呈正相關(guān),VAI與HDL-C呈負(fù)相關(guān),具有統(tǒng)計學(xué)意義(p0.05)。女性人群中年齡與FPG (r=0.253, p=0.000)、年齡與VAI (r=0.163, p=0.008)均有顯著相關(guān)性,具有統(tǒng)計學(xué)意義(p0.05)。在混雜有年齡因素時FPG與VAI相關(guān)性分析結(jié)果提示男女組中均有顯著相關(guān)性(r男=0.183 VS r女=0.141),具有統(tǒng)計學(xué)意義(p0.05)。而排除年齡因素后,男女組中FPG與VAI均無明顯相關(guān)性。④二項分類資料logistic回歸結(jié)果表明,當(dāng)WC(男性)85cm,WC(女性)80cm時,血壓異常的患病風(fēng)險OR=2.836,血脂異常的患病風(fēng)險OR=1.736,高血糖的患病風(fēng)險是OR=2.835。當(dāng)BMI≥23 kg/m2時,血壓異常的患病風(fēng)險OR=3.541,血脂異常的患病風(fēng)險OR=2.037,高血糖的患病風(fēng)險OR=1.862(p0.05)。當(dāng)BMI、WC數(shù)值均達(dá)到超重或肥胖的標(biāo)準(zhǔn)時,血壓異常、血脂異常、高血糖的患病風(fēng)險OR值均大于單一項目達(dá)標(biāo)時的OR值。結(jié)論:①在廣西柳江縣399名受檢者中,男性較女性更易患肥胖癥、血壓異常、血脂異常。②隨著BMI、WC、VAI的增加,血壓異常、高血糖、血脂異常的檢出率呈上升趨勢。③女性人群中年齡與FPG及VAI存在相關(guān)關(guān)系,VAI與血脂的相關(guān)性較男性明顯。④BMI是血壓異常(高于正常)、血脂異常的重要預(yù)測因子,WC是高血糖的重要預(yù)測因子。BMI、WC對血壓異常(高于正常)、血脂異常、高血糖的患病情況起著協(xié)同作用。
[Abstract]:Objective: to investigate the relationship between body mass index (BMI), waist circumference (WCC), visceral fat index (VAII) and blood pressure, blood glucose and blood lipid in adults in Liujiang County, Guangxi. To prevent and control hypertension, diabetes, hyperlipidemia and other obesity related chronic diseases. Methods: from December 2013 to January 2014, 681 people were investigated in Liujiang County. There was a positive correlation between blood pressure and TG, and a negative correlation between VAI and HDL-C. There was a significant correlation between VAI and blood pressure and blood lipid in female patients (P 0.05). There was a positive correlation between VAI and MAPP (mean arterial pressure) TGG and TCLDL-C were negatively correlated with HDL-C, and there was significant difference between them (p 0.05). There was a significant correlation between age and FPG 0.253, p0. 000, VAI 0.163, p0. 008) in female population, which had statistical significance (P 0. 05). The results of correlation analysis between FPG and VAI showed that there was a significant correlation between FPG and VAI in male and female groups (0.183 vs 0.141), which had statistical significance (p 0.05).
