【摘要】：目的：通過(guò)補(bǔ)腎活血法聯(lián)合糖皮質(zhì)激素治療原發(fā)性腎病綜合征的臨床研究，初步評(píng)價(jià)補(bǔ)腎活血法聯(lián)合糖皮質(zhì)激素的中西醫(yī)結(jié)合治療方案在增強(qiáng)療效、減輕毒副反應(yīng)方面的作用。 方法：本研究收集原發(fā)性腎病綜合征符合中醫(yī)辨證腎虛血瘀證患者60例，分為試驗(yàn)組30例，對(duì)照組30例。對(duì)照組予免疫抑制、降蛋白尿等西醫(yī)基礎(chǔ)治療，試驗(yàn)組在西醫(yī)基礎(chǔ)治療上加用益腎活血湯，療程為3個(gè)月。觀察臨床癥狀、體征及實(shí)驗(yàn)室檢查指標(biāo)的變化，并對(duì)癥狀及體征進(jìn)行量化評(píng)分；觀察激素引起的副作用及減量過(guò)程中出現(xiàn)的反跳復(fù)發(fā)現(xiàn)象，，作出療效評(píng)價(jià)。 結(jié)果：試驗(yàn)組在改善臨床癥狀及檢驗(yàn)指標(biāo)方面均優(yōu)于對(duì)照組；試驗(yàn)組激素副作用的發(fā)生率總體上低于對(duì)照組。試驗(yàn)組能夠提高臨床緩解率，改善臨床癥狀；降低蛋白尿的排泄，升高血白蛋白水平，降低D二聚體，改善高凝狀態(tài)；減少激素的副作用及激素減藥過(guò)程中疾病的反跳和復(fù)發(fā)。 結(jié)論：補(bǔ)腎活血法結(jié)合糖皮質(zhì)激素治療原發(fā)性腎病綜合征具有提高激素療效、減輕激素毒副反應(yīng)的作用，中西醫(yī)聯(lián)合治療方案值得進(jìn)一步研究推廣應(yīng)用。
[Abstract]:Objective: to evaluate the effect of Bushen Huoxue method combined with glucocorticoid in the treatment of primary nephrotic syndrome. Methods: 60 patients with primary nephrotic syndrome were divided into experimental group (n = 30) and control group (n = 30). The control group was treated with basic western medicine such as immunosuppression and proteinuria. The experimental group was treated with Yishen Huoxue decoction for 3 months. The changes of clinical symptoms, physical signs and laboratory indicators were observed, and the scores of symptoms and signs were quantified, and the side effects caused by hormones and the phenomenon of rebound recurrence in the course of reduction were observed, and the curative effect was evaluated. Results: the experimental group was superior to the control group in improving clinical symptoms and testing indexes, and the incidence of hormone side effects in the trial group was lower than that in the control group. The experimental group can increase the clinical remission rate and improve the clinical symptoms, reduce the excretion of proteinuria, raise the level of serum albumin, reduce the D-dimer, improve the hypercoagulability, reduce the side effects of hormones, and reduce the rebound and recurrence of diseases in the course of hormone reduction. Conclusion: the therapy of tonifying kidney and activating blood circulation combined with glucocorticoid therapy can improve the effect of hormone and reduce the toxic and side effects of hormone. The combination of traditional Chinese and western medicine is worthy of further study and application.
