【摘要】：目的探討合并組織學(xué)前列腺炎(HP)對(duì)前列腺增生患者的尿動(dòng)力學(xué)參數(shù)及尿潴留的影響。方法回顧性分析98例接受經(jīng)尿道前列腺等離子雙極電切術(shù)治療的良性前列腺增生(BPH)患者資料,根據(jù)病理診斷分為單純BPH組和BPH合并HP組,比較國(guó)際前列腺癥狀評(píng)分(IPSS)、前列腺體積(PV)、前列腺特異性抗原(PSA)、最大尿流率、最大膀胱容量(MBC)、最大逼尿肌收縮力和膀胱出口梗阻指數(shù)(BOOI)等尿動(dòng)力學(xué)指標(biāo)及膀胱逼尿肌過度活動(dòng)、尿潴留發(fā)生率間的差異。結(jié)果與單純BPH組比較,BPH合并HP組具有更高的IPSS評(píng)分(P=0.020)和PSA值(P=0.009),更高的BOOI指數(shù)(P=0.003),較小的最大膀胱容量(P=0.004),更高的膀胱逼尿肌過度活動(dòng)的發(fā)生率(P=0.041)和尿潴留發(fā)生率(P=0.010)。結(jié)論合并HP的BPH患者下尿路癥狀更重,血清PSA升高,導(dǎo)致MBC降低、膀胱出口梗阻加重和逼尿肌過度活動(dòng)的發(fā)生等尿動(dòng)力學(xué)參數(shù)改變,更易發(fā)生尿潴留。
[Abstract]:Objective to investigate the effect of (HP) combined with histologic prostatitis on urodynamic parameters and urinary retention in patients with prostatic hyperplasia. Methods the data of 98 patients with benign prostatic hyperplasia (BPH) treated by transurethral plasma bipolar resection were retrospectively analyzed. According to the pathological diagnosis, they were divided into simple BPH group and BPH combined with HP group. International prostate symptom score (IPSS), prostate volume (PV), prostate specific antigen (PSA), maximal bladder volume (MBC), maximal detrusor contractility (MBC), bladder outlet obstruction index (BOOI), and hyperactivity of bladder detrusor were compared. Differences in the incidence of urinary retention. Results compared with BPH group, the IPSS score (P0. 020) and PSA value (P0. 009), higher BOOI index (P0. 003), lower maximal bladder volume (P0. 004), higher incidence of bladder detrusor overactivity (P0. 041) and urinary retention (P0. 010) were higher in the BPH combined with HP group. Conclusion the lower urinary tract symptoms of BPH patients with HP are more serious, the serum PSA level is higher, which leads to the decrease of MBC, the increase of bladder outlet obstruction and the occurrence of detrusor hyperactivity, and the change of urodynamic parameters is more likely to occur urinary retention.
