【摘要】：目的探討全身麻醉復(fù)合硬膜外麻醉對(duì)胃癌患者術(shù)后鎮(zhèn)痛效果的影響。方法以80例行胃癌根治術(shù)的患者作為研究對(duì)象,將其根據(jù)麻醉方式不同分為對(duì)照組與觀察組,每組40例。對(duì)照組給予單純?nèi)?觀察組給予全身麻醉復(fù)合硬膜外麻醉,觀察兩組患者術(shù)后不同時(shí)間段VAS評(píng)分情況、PCIA使用次數(shù)及不良反應(yīng)發(fā)生情況等。結(jié)果觀察組術(shù)后2、4、8、12、24 h VAS評(píng)分明顯低于對(duì)照組(P0.05);觀察組48 h內(nèi)PCIA按壓次數(shù)低于對(duì)照組,鎮(zhèn)靜評(píng)分高于對(duì)照組(P0.05),不良反應(yīng)發(fā)生率為10.0%,明顯低于對(duì)照組的32.5%(P0.05)。結(jié)論胃癌患者手術(shù)時(shí)采用全身麻醉復(fù)合硬膜外麻醉,可明顯改善患者術(shù)后疼痛情況,提高術(shù)后鎮(zhèn)痛效果,減少不良反應(yīng),改善預(yù)后及生活質(zhì)量,其臨床應(yīng)用價(jià)值高。
[Abstract]:Objective to investigate the effect of general anesthesia combined with epidural anesthesia on postoperative analgesia in patients with gastric cancer. Methods 80 patients undergoing radical resection of gastric cancer were divided into control group (n = 40) and observation group (n = 40) according to different anaesthesia methods. The control group was given general anesthesia and the observation group was given general anesthesia combined with epidural anesthesia. The VAS score, the frequency of PCIA use and the occurrence of adverse reactions in the two groups were observed at different time points after operation. Results the VAS score of the observation group at 24 h was significantly lower than that of the control group at 2, 4, 8, 12 and 24 h after operation (P 0.05). The number of PCIA compression in the observation group was lower than that in the control group within 48 hours, and the sedation score was higher than that in the control group (P 0.05). The incidence of adverse reactions was 10.0%, which was significantly lower than that in the control group (32.5%). Conclusion General anesthesia combined with epidural anesthesia can significantly improve postoperative pain, improve postoperative analgesia, reduce adverse reactions, improve prognosis and quality of life in patients with gastric cancer.
