【摘要】：目的:探討蛛網(wǎng)膜下腔阻滯麻醉在老年患者腔內(nèi)泌尿外科微創(chuàng)手術(shù)中的應(yīng)用效果。方法:198例老年腔內(nèi)泌尿外科微創(chuàng)手術(shù)患者按照隨機(jī)數(shù)字表法分為觀察組(100例)和對照組(98例)。觀察組患者采用蛛網(wǎng)膜下腔阻滯麻醉;對照組患者采用硬膜外麻醉。比較兩組患者的麻醉起效時(shí)間、麻醉用藥量、手術(shù)時(shí)間、阻滯完全時(shí)間、麻醉效果滿意度及不良反應(yīng)發(fā)生情況。結(jié)果:觀察組患者的麻醉起效時(shí)間為(1.5±0.6)min、阻滯完全時(shí)間為(7.9±3.9)min,均明顯短于對照組[分別為(4.5±1.2)、(17.5±4.3)min],差異均有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組患者的麻醉用藥量為(20.2±4.8)mg,明顯低于對照組的(103.4±20.1)mg,差異有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組患者對麻醉效果的滿意率為95.0%,明顯高于對照組(69.4%),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組患者手術(shù)時(shí)間和不良反應(yīng)發(fā)生率與對照組比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:蛛網(wǎng)膜下腔阻滯麻醉的用藥量少、起效快、麻醉效果顯著,適用于老年患者腔內(nèi)泌尿外科微創(chuàng)手術(shù)。
[Abstract]:Objective: to investigate the effect of subarachnoid block anesthesia in the minimally invasive operation of endourinary surgery in elderly patients. Methods one hundred and eighty-eight elderly patients undergoing minimally invasive surgery in endourinary surgery were randomly divided into two groups: observation group (n = 100) and control group (n = 98). The patients in the observation group were anesthetized by subarachnoid block and the patients in the control group by epidural anesthesia. Anaesthesia onset time, anesthetic dosage, operation time, complete block time, anesthetic satisfaction and adverse reaction were compared between the two groups. Results: the onset time of anesthesia in the observation group was (1.5 鹵0.6) min, block complete time was (7.9 鹵3.9) min, significantly shorter than that in the control group [(4.5 鹵1.2), (17.5 鹵4.3) min, respectively], the difference was statistically significant (P0.05), the amount of anesthetic drug used in the observation group was (20.2 鹵4.8) mg, significantly lower than that in the control group (103.4 鹵20.1) mg,. The difference was statistically significant (P0.05); the satisfaction rate of the patients in the observation group was 95.0, significantly higher than that in the control group (69.4%), the difference was statistically significant (P0.05). The time of operation and the incidence of adverse reactions in the observation group were not significantly different from those in the control group (P0.05). Conclusion: subarachnoid block anesthesia has the advantages of low dosage, quick effect and remarkable anaesthesia effect. It is suitable for the minimally invasive operation of endovascular urology in elderly patients.
