【摘要】：目的:評(píng)價(jià)全身麻醉與硬膜外麻醉對(duì)老年股骨頸骨折患者髖關(guān)節(jié)置換手術(shù)的影響,探討適合老年患者行髖關(guān)節(jié)置換術(shù)的麻醉方法。方法:共91例老年股骨頸骨折患者接受髖關(guān)節(jié)置換術(shù)。其中全身麻醉組50例,硬膜外麻醉組41例。比較2種麻醉方法在手術(shù)時(shí)間、出血量、術(shù)后輸血量、術(shù)后惡心嘔吐、術(shù)后尿潴留、住院天數(shù)等方面的指標(biāo)。結(jié)果:硬膜外麻醉組患者在術(shù)后惡心嘔吐、術(shù)后輸血量、住院天數(shù)優(yōu)于全身麻醉組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);手術(shù)時(shí)間、出血量、術(shù)后尿潴留方面比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:老年股骨頸骨折患者行髖關(guān)節(jié)置換術(shù)時(shí),實(shí)施硬膜外麻醉方法與全身麻醉方法相比,硬膜外麻醉方法能降低術(shù)后惡心嘔吐發(fā)生率,減少術(shù)后輸血量,縮短住院時(shí)間,有一定的優(yōu)越性,值得推薦。
[Abstract]:Objective: to evaluate the effect of general anesthesia and epidural anesthesia on hip arthroplasty in elderly patients with femoral neck fracture and to explore the anaesthesia method for hip replacement in elderly patients. Methods: a total of 91 elderly patients with femoral neck fracture underwent hip replacement. There were 50 cases in general anesthesia group and 41 cases in epidural anesthesia group. The operative time, blood loss, blood transfusion, postoperative nausea and vomiting, postoperative urinary retention and hospital stay were compared between the two anesthesia methods. Results: the postoperative nausea and vomiting, blood transfusion and hospitalization days in epidural anesthesia group were significantly better than those in general anesthesia group (P0.05), but there was no significant difference in operation time, blood loss and postoperative urinary retention (P0.05). Conclusion: compared with general anesthesia, epidural anesthesia can reduce the incidence of nausea and vomiting, reduce the amount of blood transfusion and shorten the hospital stay in elderly patients with femoral neck fracture. Has certain superiority, is worth recommending.
