【摘要】：目的探討地佐辛與氟比洛芬酯對(duì)乳腺癌圍術(shù)期鎮(zhèn)痛效果與免疫抑制的影響。方法選取該院外科乳腺癌擇期手術(shù)患者76例,在相同麻醉誘導(dǎo)和維持用藥基礎(chǔ)上,采用隨機(jī)數(shù)字表法將患者分為觀察組(n=40)和對(duì)照組(n=36),觀察組患者麻醉誘導(dǎo)前給予地佐辛,對(duì)照組患者麻醉誘導(dǎo)前給予氟比洛芬酯,比較兩組患者圍術(shù)期鎮(zhèn)痛效果和免疫抑制的影響。結(jié)果觀察組術(shù)后3.0、6.0、12.0h視覺(jué)模擬疼痛評(píng)分(VAS)和Ramsay鎮(zhèn)靜評(píng)分明顯低于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。麻醉前,T淋巴細(xì)胞亞群(CD4+、CD8+、CD4+/CD8+)和NK細(xì)胞(CD16+CD56+)的組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),觀察組手術(shù)開(kāi)始1.5h時(shí),CD16+CD56+明顯高于對(duì)照組,手術(shù)結(jié)束后24.0h,CD4+、CD4+/CD8+和CD16+CD56+明顯高于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),且手術(shù)開(kāi)始1.5h、手術(shù)結(jié)束后24.0h的上述指標(biāo)均明顯低于麻醉前,不同時(shí)段比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),兩組患者術(shù)后不良反應(yīng)發(fā)生率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論地佐辛對(duì)乳腺癌圍術(shù)期鎮(zhèn)痛效果和免疫抑制預(yù)防作用明顯優(yōu)于氟比洛芬酯。
[Abstract]:Objective to investigate the effect of dizosin and flurbiprofen on perioperative analgesia and immunosuppression of breast cancer.Methods Seventy-six patients with breast cancer undergoing elective operation were divided into two groups: observation group (n = 40) and control group (n = 36), on the basis of the same anesthetic induction and maintenance medication. The patients in the observation group were given dizosin before anesthesia induction.Patients in the control group were treated with flurbiprofen before anesthesia induction, and the analgesic effect and immunosuppressive effect were compared between the two groups.Results the visual analogue pain score (VASS) and Ramsay sedation score in the observation group were significantly lower than those in the control group (P 0.05).There was no significant difference between the T lymphocyte subsets (CD4 / CD8 / CD8) and NK cell CD16 CD56 (P 0.05). At 1.5 h after operation, the CD16 CD56 in the observation group was significantly higher than that in the control group, and the ratio of CD4 / CD8 and CD16 CD56 in the observation group was significantly higher than that in the control group at 24.0h after operation.The difference between the two groups was statistically significant (P 0.05), and the above indexes were significantly lower than those before anesthesia at the beginning of operation 1.5 hours and 24.0h after operation.There was no significant difference in the incidence of postoperative adverse reactions between the two groups (P 0.05).Conclusion the analgesic effect and immunosuppressive effect of dizosin on breast cancer is better than flurbiprofen ester.
