【摘要】：目的:研究慢性鼻-鼻竇炎伴鼻息肉(CRSwNP)患者鼻阻力及肺功能特點(diǎn),探討鼻阻力對(duì)CRSwNP患者鼻腔通氣功能的評(píng)價(jià)作用,以及鼻內(nèi)鏡手術(shù)對(duì)CRSwNP患者肺功能的影響。方法:選取符合研究標(biāo)準(zhǔn)的50例CRSwNP患者為研究對(duì)象,均在全身麻醉下按照Messerklinger術(shù)式進(jìn)行鼻內(nèi)鏡手術(shù),手術(shù)范圍根據(jù)術(shù)前CT顯示病變范圍及鼻內(nèi)鏡檢查所見(jiàn)確定,圍手術(shù)期行鼻內(nèi)糖皮質(zhì)激素、頭孢類(lèi)或青霉素類(lèi)抗生素、鼻腔沖洗等治療。主要觀察指標(biāo)包括術(shù)前及術(shù)后1周、3個(gè)月、6個(gè)月患者視覺(jué)模擬量表(VAS)、鼻內(nèi)鏡Lund-Kennedy評(píng)分、鼻阻力、肺通氣功能即第1秒用力呼氣容積(FEV1)、用力肺活量(FVC)、FEV1/FVC比值及峰值呼氣流速(PEF)。結(jié)果:CRSwNP患者VAS評(píng)分、鼻內(nèi)鏡Lund-Kennedy評(píng)分及鼻阻力之間存在顯著正相關(guān)(P0.05),而其VAS評(píng)分、鼻內(nèi)鏡Lund-Kennedy評(píng)分及鼻阻力與肺功能指標(biāo)FEV1、FVC及PEF之間均存在顯著負(fù)相關(guān)(P0.05)。CRSwNP患者行以鼻內(nèi)鏡手術(shù)為主的綜合治療后其VAS評(píng)分、鼻內(nèi)鏡Lund-Kennedy評(píng)分及鼻阻力檢測(cè)值均較術(shù)前顯著降低,均差異有統(tǒng)計(jì)學(xué)意義(P0.05);行鼻內(nèi)鏡手術(shù)后CRSwNP患者肺功能指標(biāo)FEV1、FVC、FEV1/FVC及PEF較術(shù)前均顯著升高,均差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:鼻阻力可以客觀及可靠地反映CRSwNP患者鼻塞程度及其鼻內(nèi)鏡手術(shù)后鼻功能的恢復(fù)情況,且檢測(cè)方法簡(jiǎn)便易行。鼻內(nèi)鏡手術(shù)可有效改善CRSwNP患者的肺通氣功能,可為防治CRS相關(guān)下呼吸道疾病提供一定的臨床參考。
[Abstract]:Objective: to study the characteristics of nasal resistance and pulmonary function in patients with chronic rhinosinusitis with nasal polyp (CRSWNP), and to explore the evaluation of nasal ventilation function in patients with CRSwNP. Methods: 50 CRSwNP patients who met the standard of the study were selected to perform endoscopic sinus surgery under general anesthesia according to the Messerklinger procedure under general anesthesia, and the effects of endoscopic sinus surgery on lung function in patients with CRSwNP were studied. The scope of operation was determined according to preoperative CT findings and endoscopic findings. Perioperative use of glucocorticoids, cephalosporins or penicillin antibiotics was performed during the perioperative period. The main parameters included visual analogue scale before and after 1 week, 3 months and 6 months after operation, nasal endoscopy Lund-Kennedy score, nasal resistance. Forced expiratory volume (FEV1), forced vital capacity (FV) / FEV1 / FVC / FEV1 / FVC and peak expiratory flow rate (PEFV) were measured. Results there was a significant positive correlation among VAS score, Lund-Kennedy score of nasal endoscope and nasal resistance in patients with 1% CRSwNP, while the VAS score was significantly correlated with FEV1 / FVC ratio of FEV1 / FVC / FEV1 / FVC / FEV1 / FVC / FVC ratio. There was a significant negative correlation between Lund-Kennedy score, nasal resistance and pulmonary function index FEV1FVC and PEF. The VAS score, Lund-Kennedy score and nasal resistance were significantly lower in patients with nasal endoscopic surgery after comprehensive treatment than those before operation. The pulmonary function indexes of patients with CRSwNP after endoscopic sinus surgery (CRSwNP) were significantly higher than those before operation (FEV1 / FVC / FEV1 / FVC and PEF). Conclusion: nasal resistance can objectively and reliably reflect the degree of nasal congestion and the recovery of nasal function in patients with CRSwNP after endoscopic surgery. Endoscopic sinus surgery can effectively improve the pulmonary ventilation function of patients with CRSwNP and provide some clinical reference for the prevention and treatment of CRS related lower respiratory diseases.
