【摘要】：目的:探究自控硬膜外麻醉鎮(zhèn)痛在分娩中對(duì)產(chǎn)程和胎兒的影響。方法:選擇我院婦產(chǎn)科2015年1月至2015年12月收治的72例初產(chǎn)婦,根據(jù)分娩過程有無實(shí)施鎮(zhèn)痛分為對(duì)照組和觀察組,觀察兩組產(chǎn)婦的分娩效果和出血量,比較兩組產(chǎn)婦產(chǎn)程各階段的VAS評(píng)分、分娩方式、產(chǎn)后出血量及新生兒Apgar評(píng)分。結(jié)果:觀察組產(chǎn)婦產(chǎn)程各階段的VAS評(píng)分和總產(chǎn)程時(shí)間明顯低于對(duì)照組(P0.05),具有統(tǒng)計(jì)學(xué)意義;觀察組產(chǎn)婦陰道分娩率明顯高于對(duì)照組,剖宮產(chǎn)率低于對(duì)照組(P0.05),在產(chǎn)后2h出血量及新生兒Apgar評(píng)分方面不存在顯著性差異,不具有統(tǒng)計(jì)學(xué)意義。結(jié)論:在產(chǎn)婦的分娩過程中,采用自控硬膜外麻醉鎮(zhèn)痛分娩可以提高產(chǎn)婦的自然分娩率,降低剖宮產(chǎn)率,鎮(zhèn)痛效果確切,可有效緩解產(chǎn)婦的疼痛;根據(jù)產(chǎn)婦產(chǎn)程進(jìn)展及時(shí)調(diào)整鎮(zhèn)痛泵注藥速度,可進(jìn)一步縮短產(chǎn)婦的產(chǎn)程時(shí)間,減少難產(chǎn)、產(chǎn)后大出血等不良反應(yīng)的發(fā)生,具有較好的控制性和安全性,值得在臨床上推廣使用。
[Abstract]:Objective: to investigate the effect of patient-controlled epidural anesthesia and analgesia on labor process and fetus during delivery. Methods: 72 cases of primipara were selected from January 2015 to December 2015 in our department of gynecology and obstetrics. According to whether or not the labor analgesia was performed, the control group and the observation group were divided into two groups. The delivery effect and bleeding volume of the two groups were observed, and the VAS score and delivery mode of each stage of labor were compared between the two groups. Results: the VAS score and total duration of labor in the observation group were significantly lower than those in the control group (P 0.05). The rate of vaginal delivery in the observation group was significantly higher than that in the control group, and the rate of cesarean section was lower than that in the control group (P 0.05). There was no significant difference in the amount of blood loss at 2 hours postpartum and the Apgar score of newborn. Conclusion: the spontaneous delivery rate of parturient can be increased and the rate of cesarean section can be reduced by using self-controlled epidural anesthesia in the process of parturition. It can effectively relieve the pain of parturient; According to the progress of labor, adjusting the speed of injecting analgesic pump in time can further shorten the time of delivery, reduce the occurrence of adverse reactions such as dystocia, postpartum hemorrhage and so on. It has good control and safety. It is worth popularizing in clinic.
