【摘要】：研究目的： 探討及研究下頜骨粉碎性骨折堅強內(nèi)固定術后，頜間牽引對咬合關系的影響。 研究對象： 選取山西醫(yī)科大學第一醫(yī)院口腔頜面外科自2011年9月至2013年12月收治的68例下頜骨粉碎性骨折患者，男性37例，女性31例�；颊吣挲g18~56歲，平均年齡38.6±6.7歲。 研究方法： 對68例下頜骨粉碎性骨折患者行堅強內(nèi)固定治療后，其中40例患者能很好的遵醫(yī)囑，，根據(jù)患者咬合關系恢復情況，橡皮圈頜間牽引1~2周，即A組；28例由于自身原因，未能遵醫(yī)囑行頜間牽引治療，即B組。選取術后兩個觀察點，分別對兩組患者的療效比較分析。 研究結果： 在本研究兩個觀察點A組患者咬合關系的恢復均明顯好于B組患者，且兩組療效結果，經(jīng)統(tǒng)計學分析后，有統(tǒng)計學意義（P＜0.05）。 研究結論： 適當?shù)念M間牽引使得下頜骨粉碎性骨折患者手術治療后咬合關系得以進一步矯正，從而提高下頜骨粉碎性骨折治療的質量。而許多下頜骨粉碎性骨折患者治療后咬合關系出現(xiàn)紊亂，主要是由于沒有有效的頜間牽引，微小范圍的錯頜未能矯正，這一步驟的缺失很可能造成患者永久的咬合紊亂。
[Abstract]:Objective: to investigate the effect of intermaxillary traction on occlusion after rigid internal fixation of comminuted mandibular fractures. Participants: Sixty-eight patients with comminuted mandibular fractures, 37 males and 31 females, were treated in oral and maxillofacial surgery of the first Hospital of Shanxi Medical University from September 2011 to December 2013. The average age of the patients was 38.6 鹵6.7 years. Methods: 68 patients with comminuted mandibular fractures were treated with rigid internal fixation, 40 of them were able to follow the doctor's instructions. According to the recovery of the occlusion relationship of the patients, the rubber band intermaxillary traction was done for 1 week, that is, group A; 28 cases were not treated with intermaxillary traction according to the doctor's instructions, I. E. group B, for their own reasons. Two observation points were selected to compare and analyze the curative effect of the two groups. Results: the recovery of occlusal relationship in group A was significantly better than that in group B in the two observation points in this study, and the curative effect of two groups was statistically significant after statistical analysis (P < 0.05). Conclusion: proper intermaxillary traction can further correct the occlusal relationship in patients with comminuted mandibular fractures and improve the quality of treatment of comminuted mandibular fractures. However, many patients with comminuted mandibular fracture have disorder of occlusal relationship after treatment, mainly due to the absence of effective intermaxillary traction and the failure of minor malocclusion. The absence of this step may lead to permanent occlusal disorder.
