【摘要】：目的針對(duì)上頜竇底剩余牙槽骨高度(residual bone height,RBH)2~3 mm的患者行上頜竇底外提升同期種植體植入術(shù)的病例研究報(bào)道較少。文中旨在評(píng)價(jià)應(yīng)用上頜竇底外提升同期種植體植入術(shù)修復(fù)上頜后牙區(qū)嚴(yán)重骨量不足病例的臨床效果。方法回顧性分析2012年1月至2013年1月在常州市口腔醫(yī)院種植中心行上頜竇底外提升同期種植體植入術(shù)的42例患者的臨床資料,共植入85顆種植體。術(shù)前錐形束計(jì)算機(jī)體層掃描測(cè)量上頜后牙種植區(qū)剩余牙槽骨高度,其中RBH為2~3 mm的53枚種植體為試驗(yàn)組,32顆RBH在4~5 mm的種植體為對(duì)照組。85枚種植體中單顆種植體為單冠組(19顆),≥2顆種植體為橋體組(66顆)。術(shù)中采用超聲骨刀行上頜竇底外提升同期種植體植入術(shù),并植入自體骨+人工骨粉混合物行骨再生術(shù),修復(fù)前測(cè)量種植體穩(wěn)定性系數(shù)(Implant stability Quotient,ISQ),術(shù)后2年測(cè)量牙槽嵴頂至上頜竇底的高度,隨訪24個(gè)月,觀察臨床療效。結(jié)果試驗(yàn)組和對(duì)照組術(shù)后24個(gè)月骨提升高度、ISQ比較差異有統(tǒng)計(jì)學(xué)意義[(7.755±0.376)vs(6.658±0.415)mm;(74.444±1.462)vs(73.058±1.477),P0.05]。logistic回歸分析顯示術(shù)后24個(gè)月骨提升高度的OR值為1.248(95%CI:0.242~6.427)。種植體提升高度對(duì)種植體的穩(wěn)定性的影響無(wú)統(tǒng)計(jì)學(xué)意義(P=0.149)。試驗(yàn)組的種植體存留率與對(duì)照組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(98.11%vs 100%,P=0.953)。單冠組種植體存留率與橋體組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(94.74%vs 100%,P=0.189)。術(shù)后24個(gè)月復(fù)查,試驗(yàn)組、對(duì)照組種植體周圍骨均整合良好并成功負(fù)載,種植體頸部骨少量吸收,種植體穩(wěn)定。結(jié)論上頜竇外提升同期種植體植入術(shù)應(yīng)用于上頜后牙區(qū)嚴(yán)重骨量不足的種植修復(fù)病例臨床效果滿意。
[Abstract]:Objective to study the patients with 2mm (residual bone height of residual alveolar bone in the maxillary sinus floor undergoing simultaneous implant implantation with exasperation of the maxillary sinus floor. The purpose of this study was to evaluate the clinical effect of simultaneous implant implantation in the repair of severe bone deficiency in maxillary posterior teeth. Methods from January 2012 to January 2013, the clinical data of 42 patients who underwent the simultaneous implant implantation in the implant center of Changzhou Stomatological Hospital were analyzed retrospectively. A total of 85 implants were implanted. The height of residual alveolar bone in maxillary posterior dental implant was measured by computer tomography of conical bundle before operation.
