【摘要】：目的:觀察布托菲諾聯(lián)合鹽酸丁卡因膠漿輔助導尿?qū)θ轭M面整形術后男性患者導尿管源性膀胱不適(cather-related bladder discomfort,CRBD)的影響。方法:擇期全麻頜面整形術男性患者90例,隨機分為3組:傳統(tǒng)導尿組(A組)、鹽酸丁卡因膠漿導尿組(B組)、布托菲諾聯(lián)合鹽酸丁卡因膠漿導尿組(C組)。分別在手術結(jié)束拔除氣管導管即刻(0h)和拔管后1、2、3、6h記錄患者平均動脈壓,心率以及手指血氧飽和度的改變;進行CRBD評分以及蘇醒期躁動評分。結(jié)果:與A組相比,拔除氣管導管即刻0h和拔管后1h,B、C兩組CRBD評分以及蘇醒期躁動評分均明顯降低(P0.05);與B組相比,拔除氣管導管即刻0h和拔管后1h,C組CRBD評分以及蘇醒期躁動評分降低,血液動力學平穩(wěn),差異有統(tǒng)計學意義(P0.05),3組患者手指血氧飽和度均在96%以上,差異無統(tǒng)計學意義。結(jié)論:布托菲諾聯(lián)合鹽酸丁卡因膠漿輔助導尿有效減少全麻蘇醒時和蘇醒后1h時CRBD癥狀的發(fā)生,明顯減少蘇醒期躁動。
[Abstract]:Objective: to observe the effect of butorfinol combined with tetracaine hydrochloride glue on cather-related bladder discompost bladder discomfort (CRBD) in male patients after general anesthesia and maxillofacial surgery. Methods: 90 male patients undergoing elective general anesthesia and maxillofacial surgery were randomly divided into three groups: traditional catheterization group (group A), tetracaine hydrochloride gel catheterization group (group B) and butorfinol combined with tetracaine hydrochloride gel catheterization group (group C). The mean arterial pressure, heart rate and finger oxygen saturation were recorded immediately (0 h) after extubation and 3 h after extubation, CRBD score and restlessness score during recovery were performed. Results: compared with group A, CRBD scores and restlessness scores in group B and B were significantly lower than those in group A at 0 h after extubation and 1 hour after extubation (P0.05), compared with group B, CRBD scores were significantly lower than those in group B (P < 0.05). The CRBD score and restlessness score in group C were decreased at 0 h after extubation and 1 hour after extubation, and the hemodynamics was stable. The difference was statistically significant (P0.05) the oxygen saturation of fingers was above 96% in group C (P0.05), the difference was not statistically significant. Conclusion: butorfinol combined with tetracaine hydrochloride glue assisted catheterization can effectively reduce the occurrence of CRBD symptoms and reduce restlessness during general anesthesia and 1 hour after recovery.
