【摘要】：目的:研究脂代謝紊亂的牙列缺損患者行種植義齒修復(fù)后牙周及種植體周的檢查、唾液中炎性因子表達(dá)、細(xì)菌的相對(duì)量和種植體穩(wěn)定性,分析脂代謝紊亂患者的種植預(yù)后狀況。方法:將脂代謝紊亂的牙列缺損患者設(shè)為實(shí)驗(yàn)組,血脂健康的牙列缺損患者設(shè)為對(duì)照組,在種植前,種植后4周、12周、24周收集患者唾液,ELISA方法檢測(cè)唾液中的白細(xì)胞介素6(IL-6)、白細(xì)胞介素1β(IL-1β)、腫瘤壞死因子α(TNF-α)表達(dá),RT-PCR檢測(cè)牙齦卟啉單胞菌(Porphyromonas gingivalis,Pg)、福賽斯坦納菌(Tannerella forsythia,TF)、具核梭桿菌(Fusobacterium nucleatum,Fn)的相對(duì)量,Osstell ISQTM共振頻率無(wú)線分析儀檢測(cè)種植體的穩(wěn)定性,并用Florida探針檢測(cè)牙周狀況。結(jié)果:2組的種植后探診深度(PD)、出血指數(shù)(BI)、共振頻率無(wú)線分析儀檢測(cè)種植體穩(wěn)定性數(shù)ISQ值無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。各組種植前,種植后4周、12周ISQ值、IL-6、IL-1β、TNF-α表達(dá)比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。種植前實(shí)驗(yàn)組IL-6、IL-1β、TNF-α的表達(dá)高于對(duì)照組(P0.05);而種植后24周實(shí)驗(yàn)組與對(duì)照組種植前IL-6、IL-1β、TNF-α的表達(dá)無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。Pg、Tf、Td、Fn相對(duì)SQ值四次檢查結(jié)果逐漸減少,差異有統(tǒng)計(jì)學(xué)意義(P0.05),Td相對(duì)SQ值四次檢查結(jié)果無(wú)差異(P0.05),實(shí)驗(yàn)組和對(duì)照組Pg、Tf、Fn、Td相對(duì)SQ值結(jié)果無(wú)差異(P0.05)。結(jié)論:種植術(shù)后脂代謝紊亂患者同血脂健康者一樣有良好的種植體穩(wěn)定性,即脂代謝紊亂患者同樣可以取得良好的種植預(yù)后。
[Abstract]:Objective: to study the periodontal and periimplant periodontal examination, the expression of inflammatory factors in saliva, the relative quantity of bacteria and the stability of implant in patients with dyslipidemic dentition, and to analyze the prognosis of implant in patients with dyslipidemia. Methods: the patients with dyslipidemia were divided into experimental group and control group. Detection of Interleukin-6, IL-1 尾, TNF- 偽 expression in saliva by Elisa for detecting Porphyromonas gingivalis PgG, Tannerella forsythia TFN, Clostridium gingivalis by RT-PCR The relative quantity of Fusobacterium nucleatum Fn. was used to detect the stability of implants by Osstell ISQTM resonance frequency wireless analyzer. Periodontal status was detected with Florida probe. Results there was no significant difference in the number of ISQ between the two groups (P 0.05), such as the depth of probe after implantation, the bleeding index and the index of BII. The ISQ of implant stability was not significantly different from that of the control group (P < 0.05). Before and 4 weeks after implantation, the ISQ value and the expression of IL-1 尾 -TNF- 偽 were significantly higher in each group than in the control group (P 0.05). The expression of IL-6, IL-1 尾 and TNF- 偽 in the experimental group was higher than that in the control group before implantation, but there was no significant difference in the expression of IL-6, IL-1 尾 and TNF- 偽 between the experimental group and the control group 24 weeks after implantation. There was no significant difference in the four times of P0.05Td relative sq value between the experimental group and the control group, but there was no difference between the experimental group and the control group. Conclusion: the patients with dyslipidemia after implantation have the same good implant stability as those with healthy blood lipids, that is, the patients with dyslipidemia can also obtain a good prognosis of implantation.
