不同材料固定局部義齒患者行頭頸部MRI檢查的成像效果和安全性比較

【摘要】:目的:采用不同材料制作固定局部義齒,并行頭頸部磁共振成像(MRI)檢查,探討其對MRI的影響。方法:招募1例符合條件的志愿者,分別選擇鈷鉻合金、金鉑合金和二氧化鋯作為內(nèi)冠材料進行固定局部義齒修復。將3種不同內(nèi)冠材料固定局部義齒就位于患者口內(nèi),進行2種序列(T1-TSE和T2-TSE)MRI掃描。以不戴有修復體的掃描圖像作為對照,觀察偽影的形狀及圖像信號的變化,檢測3組不同材料義齒偽影累及層數(shù),檢測3種材料在冠狀位、矢狀位和軸位圖像中偽影的最大直徑,觀察固定局部義齒的安全性。結(jié)果:各組修復體均產(chǎn)生偽影,其表現(xiàn)形式為信號的強弱變化和圖像形狀變化;偽影范圍在所累及的中心層面最大,在矢狀位、冠狀位和軸位圖像上有不同的表現(xiàn)形式。鈷鉻合金組義齒偽影累及層數(shù)明顯多于金鉑合金組和二氧化鋯組,偽影均限于修復側(cè)頜面部,未累及頸椎及顱腦部。鈷鉻合金組義齒累及或輕微累及所有鄰近組織,金鉑合金組和二氧化鋯組義齒僅輕微累及下頜牙、舌肌及上頜牙冠。鈷鉻合金組義齒產(chǎn)生的偽影最大直徑大于金鉑合金組和二氧化鋯組(P0.05)。修復體未見移位,患者無自覺不適。結(jié)論:口腔固定局部義齒在MRI檢查時會產(chǎn)生偽影。金鉑合金或二氧化鋯內(nèi)冠修復體可明顯降低偽影的影響。口內(nèi)妥善粘接的固定修復體對MRI檢查患者不產(chǎn)生安全影響。
[Abstract]:Objective: to investigate the effect of fixed partial denture made from different materials on MRI by magnetic resonance imaging (MRI) of head and neck. Methods: one eligible volunteer was recruited to select cobalt-chromium alloy. Au-Pt alloy and zirconia were used as internal crown materials for fixed partial denture. Three different internal crown materials were used to fix partial denture in the patient's mouth. T1-TSE and T2-TSEE MRI scans were performed. The shape of artifacts and the changes of image signals were observed, and the number of layers involved in three groups of denture artifacts were detected, and the coronal position of three kinds of materials were detected. The maximum diameter of artifact in sagittal and axial images and the safety of fixed partial denture were observed. The range of artifacts was the largest at the central level involved, and had different manifestations in sagittal, coronal and axial images. The number of layers involved in denture artifacts in cobalt-chromium alloy group was significantly higher than that in gold platinum alloy group and zirconia group. Artifact was limited to the restoration of lateral maxillofacial region without cervical vertebrae or craniocerebral involvement. Cobalt chromium alloy dentures involved or slightly affected all adjacent tissues, while gold platinum alloy and zirconia dentures only slightly affected mandibular teeth. Tongue muscles and maxillary crowns. The maximum artifact diameter of the cobalt-chromium alloy denture was larger than that of the gold-platinum alloy group and the zirconia group P0.05.The prosthesis was not displaced. Conclusion: artifact will occur during MRI examination. Gold platinum alloy or zirconia internal crown prosthesis can obviously reduce the effect of artifact. The properly bonded fixed prosthesis in oral cavity can be used for MRI examination. No safety effects were observed.
