【摘要】：目的：比較腮腺淺葉部分切除術與腮腺淺葉切除術對術后腮腺吸收和分泌功能的影響。方法：前瞻性研究2012年12月至2013年12月四川省腫瘤醫(yī)院術后診斷為多形性腺瘤患者,根據(jù)患者腫瘤的大小情況,將病人分為腮腺淺葉部分切除術組(n=33)和腮腺淺葉切除術組(n=18),采用唾液腺顯像的方法,以正常側作為對照,研究兩組患者的術前患側腮腺的吸收率和分泌指數(shù),術后隨訪1年(隨訪分術后3月,6月,12月,共3次),連續(xù)性比較患側腮腺的吸收率和分泌指數(shù)。結果：共有51例患者進入該臨床實驗,隨訪1年內(nèi),兩組患者均無復發(fā)或需要二次手術病例。經(jīng)過術前唾液腺顯像,51例患者,以正常側腮腺作為對照,患側腮腺的吸收率高于正常側(t=3.675;P=0.011),差異有統(tǒng)計學意義；但患側腮腺與正常側腮腺的分泌指數(shù)無統(tǒng)計學差異(t=1.968;P=0.052)； 且經(jīng)過手術后,所有患者患側腮腺的吸收率和分泌指數(shù)均低于正常側,差異有統(tǒng)計學意義(P0.05)；腮腺淺葉部分切除術組術后3月后的吸收率和術后6月后的分泌指數(shù)均高于腮腺淺葉切除術組,差異有統(tǒng)計學意義(P0.05)。結論：腮腺淺葉部分切除術在保留患者腮腺吸收與分泌功能方面優(yōu)于腮腺淺葉切除術。
[Abstract]:Objective: to compare the effects of partial parotidectomy and superficial parotid lobectomy on the absorption and secretion of parotid gland after operation. A prospective study from December 2012 to December 2013 in Sichuan Provincial Cancer Hospital diagnosed as pleomorphic adenoma. According to the tumor size of the patients, the patients were divided into two groups: the superficial parotid lobectomy group and the parotid gland superficial lobectomy group. The salivary gland imaging was performed and the normal side was used as the control. The absorptivity and secretion index of the affected parotid gland in both groups were studied. The patients were followed up for one year (three times: March, June, December). Results: a total of 51 patients were enrolled in the clinical trial and followed up for one year. There were no recurrence or secondary operation cases in both groups. After preoperative salivary gland imaging, the absorptivity of the affected parotid gland was 3.675 higher than that of the normal parotid gland. The difference was statistically significant. However, there was no significant difference in secretory index between the affected parotid gland and the normal parotid gland. P0. 052; After operation, the absorptivity and secretion index of parotid gland in all patients were lower than those in normal side, and the difference was statistically significant (P 0.05). The absorptivity after March and the secretion index after June were higher in the superficial parotid lobectomy group than in the parotid superficial lobectomy group. Conclusion: partial parotidectomy is superior to superficial parotid lobectomy in preserving the function of absorption and secretion of parotid gland.
