【摘要】：目的: 探討江西漢族人群微衛(wèi)星D6S274基因多態(tài)性與精神分裂癥認知功能、陽性和陰性癥狀的關(guān)系。 方法: 研究組174例住院患者（男100例，女74例），漢族，年齡18～40歲，符合國際疾病分類第10版（ICD-10）精神分裂癥的診斷標準，3個月內(nèi)未應(yīng)用促智劑或未做電休克治療(ECT)，病情基本控制，能配合完成認知功能測驗；同期選取與之性別、年齡、種族和文化程度均衡可比的174例健康體檢者為正常對照組(男98例，女76例)。所有入選研究對象對本研究知情同意。采用蒙特利爾量表（MoCA）對所有入選研究對象進行認知功能評定，采用陽性與陰性癥狀量表(PANSS)對患者精神癥狀進行評定。所有數(shù)據(jù)使用SPSS16.0軟件進行t檢驗、單因素方差分析、組間兩兩比較、多元逐步線性回歸、直線相關(guān)等統(tǒng)計分析。檢驗水準=0.05。 結(jié)果: 1.研究組和對照組的MoCA總分、看圖命名、視空間及執(zhí)行功能、記憶及延遲記憶、語言、注意及抽象思維等得分比較，差異均有統(tǒng)計學(xué)意義(P＜0.05)；研究組中男女患者的看圖命名和注意得分比較，差異有統(tǒng)計學(xué)意義(P＜0.05)， MoCA總分、視空間及執(zhí)行功能、記憶及延遲記憶、語言、抽象思維和定向得分比較，差異無統(tǒng)計學(xué)意義（P＞0.05）。 2.研究組中不同基因型組間，MoCA量表的各級測試得分及PANSS量表的陰性癥狀分、PANSS總分比較，差異均有統(tǒng)計學(xué)意義(P＜0.05)；組間兩兩比較， D6S274的172bp患者的視空間/執(zhí)行功能、看圖命名、記憶及延遲記憶、注意、語言、抽象思維及定向得分低于其它基因型(P＜0.05)，PANSS量表陰性癥狀分和總分高于其它各基因型（P＜0.05）。 3.多元線性回歸結(jié)果顯示，D6S274的172bp與患者的視空間/執(zhí)行功能、看圖命名、記憶及延遲記憶、注意和語言方面以及PANSS總分、陰性癥狀分有關(guān)。 4.直線相關(guān)分析顯示，精神分裂癥患者的認知功能損害與陰性癥狀相關(guān)。 結(jié)論 1.精神分裂癥患者存在廣泛的認知功能損害，主要表現(xiàn)在視空間/執(zhí)行功能、看圖命名、記憶及延遲記憶、注意、語言及抽象思維方面，且無性別差異。 2. D6S274多態(tài)性可能與精神分裂癥認知功能的視空間/執(zhí)行功能、看圖命名、記憶及延遲記憶、注意和語言損害有關(guān)。 3. D6S274多態(tài)性可能影響精神分裂癥的陰性癥狀，但與陽性癥狀可能無關(guān)。 4.精神分裂癥認知功能損害與陰性癥狀呈負相關(guān)。
[Abstract]:Objective: to investigate the relationship between polymorphism of microsatellite D6S274 gene and cognitive function, positive and negative symptoms in Jiangxi Han population. Methods: the study group consisted of 174 inpatients (100 males and 74 females), the Han nationality, aged 1840 years, who were in accordance with the criteria for the diagnosis of schizophrenia in the 10th edition of the International Classification of Diseases (ICD-10). Within 3 months, patients with (ECT), were not treated with intelligence enhancer or electroshock therapy, and they were able to complete cognitive function test. At the same time, 174 healthy controls (male 98, female 76) were selected as the control group, with a balance of sex, age, race and education. All the selected subjects had informed consent to the study. All subjects were assessed with Montreal scale (MoCA) and positive and negative symptom scale (PANSS). All data were analyzed with SPSS16.0 software for t test, single factor analysis of variance, comparison between groups, multivariate stepwise linear regression, linear correlation and so on. Test level = 0.05. Results: 1. The scores of MoCA, visual space and executive function, memory and delayed memory, language, attention and abstract thinking were significantly different between the study group and the control group (P < 0. 05). In the study group, there was a significant difference in the scores of the nomenclature and attention (P < 0. 05), MoCA), the scores of visual space and executive function, memory and delayed memory, language, abstract thinking and orientation. The difference was not statistically significant (P > 0.05). 2. In the study group, there were significant differences in the scores of all levels of MoCA scale, the negative symptom score of PANSS scale and the total score of PANSS among different genotypic groups (P < 0. 05). The scores of visual space / executive function, image naming, memory and delayed memory, attention, language, abstract thinking and orientation in 172bp patients with D6S274 were lower than those of other genotypes (P < 0. 05). The negative symptom score and total score of PANSS scale were higher than those of other genotypes (P < 0. 05). 3. Multiple linear regression analysis showed that 172bp of D6S274 was related to visual space / executive function, image naming, memory and delayed memory, attention to language, total score of PANSS, negative symptom score. 4. Linear correlation analysis showed that cognitive impairment was associated with negative symptoms in schizophrenic patients. Conclusion 1. There were extensive cognitive impairment in schizophrenic patients, mainly in visual space / executive function, picture naming, memory and delayed memory, attention, language and abstract thinking, and there was no gender difference. 2. D6S274 polymorphism may be associated with visual space / executive function of cognitive function, picture naming, memory and delayed memory, attention and language impairment in schizophrenia. 3. D6S274 polymorphism may affect the negative symptoms of schizophrenia, but may not be associated with positive symptoms. 4. There was a negative correlation between cognitive impairment and negative symptoms in schizophrenia.
