廣泛性焦慮障礙患者情緒語義無意識(shí)加工的事件相關(guān)電位研究

【摘要】:目的:廣泛性焦慮障礙(Generalized anxiety disorder, GAD)又稱慢性焦慮,臨床以對(duì)日常事件或活動(dòng)過分的、不可控制的擔(dān)心為特征表現(xiàn)。盡管目前廣泛性焦慮障礙的發(fā)病機(jī)制研究尚未取得一致性的結(jié)論,但是廣泛性焦慮障礙的信息加工理論以及行為學(xué)、影像學(xué)結(jié)果均提示,情緒信息的自動(dòng)加工或無意識(shí)加工異常可能是廣泛性焦慮障礙的重要發(fā)病機(jī)制之一。 信息無意識(shí)加工的特點(diǎn)為無或極少意識(shí)參與的信息加工過程,,不占用注意資源,無需意識(shí)的發(fā)動(dòng)。閾下啟動(dòng)范式是研究無意識(shí)加工的一種廣泛的實(shí)驗(yàn)范式。由于廣泛性焦慮障礙患者擔(dān)心是一種語義層面的思維活動(dòng),即患者反復(fù)的不停的思考某些事件會(huì)產(chǎn)生的不良后果。因此本實(shí)驗(yàn)采用閾下情緒語義啟動(dòng)范式。事件相關(guān)電位(event-related potentials, ERP)技術(shù)可以將時(shí)間精確到毫秒,能動(dòng)態(tài)的反映大腦神經(jīng)心理活動(dòng)的時(shí)間進(jìn)程,是研究無意識(shí)加工的最佳技術(shù)手段。因此,本研究的目的是利用事件相關(guān)電位方法探討廣泛性焦慮障礙患者情緒語義無意識(shí)加工的特點(diǎn),并進(jìn)一步分析不同情緒內(nèi)容及腦區(qū)分布有無特異性。 方法:研究對(duì)象包括12例確診的廣泛性障礙患者以及年齡、性別、教育程度相匹配的12名正常對(duì)照。所有被試均經(jīng)過標(biāo)準(zhǔn)化結(jié)構(gòu)訪談程序(SCID)的嚴(yán)格篩查,并完成漢密爾頓焦慮量表(HAMA)及17項(xiàng)漢密爾頓抑郁量表(HAMD-17)評(píng)分。本研究采用閾下情緒語義啟動(dòng)范式,情緒啟動(dòng)詞語分為負(fù)性、正性和中性三種情緒類型,實(shí)驗(yàn)任務(wù)是對(duì)中性圖片進(jìn)行情感等級(jí)評(píng)分。情緒啟動(dòng)詞語和任務(wù)圖片均選自中國情緒材料數(shù)據(jù)庫。應(yīng)用64導(dǎo)Neuroscan腦電生理儀來記錄事件相關(guān)電位。然后,對(duì)記錄的腦電數(shù)據(jù)進(jìn)行離線分析。分別測(cè)量額區(qū)、頂區(qū)、枕區(qū)及中央?yún)^(qū)不同情緒啟動(dòng)詞語誘發(fā)的P1成分和P3成分的平均波幅及潛伏期。采用重復(fù)測(cè)量方差分析(Repeated measure ANOVA)對(duì)兩組ERP成分的平均波幅和潛伏期進(jìn)行統(tǒng)計(jì)分析。 結(jié)果: 1.兩組被試在不同類別情緒啟動(dòng)詞語條件下的反應(yīng)時(shí)(F(1,22)=0.374,p>0.05)和情感等級(jí)評(píng)分(F(1,22)=1.419, p>0.05)未發(fā)現(xiàn)顯著的組別差異及兩者的焦慮。 2.兩組被試P1、P3的平均波幅均存在大腦半球主效應(yīng)(p<0.05)和電極位置主效應(yīng)(p<0.05)。左側(cè)半球平均波幅大于右側(cè)半球,額中央?yún)^(qū)平均波幅大于頂枕區(qū);兩組被試P1、P3的平均波幅未發(fā)現(xiàn)組別和情緒類型主效應(yīng)及兩者間的交互效應(yīng)(p>0.05)。 3.兩組被試P1和P3的潛伏期未發(fā)現(xiàn)組別和情緒類別的主效應(yīng)(p>0.05),存在半球和電極位置的主效應(yīng)(p<0.05)。左半球P1和P3的潛伏期較右半球延遲,額中央?yún)^(qū)P1和P3的潛伏期較枕頂區(qū)延遲。P3的潛伏期存在組別×情緒×半球的交互效應(yīng)(p<0.05),即健康對(duì)照組左半球負(fù)性詞語的潛伏期較正性詞語延遲,而GAD組左半球負(fù)性和正性詞語的潛伏期無顯著差異。 結(jié)論: 1.未發(fā)現(xiàn)廣泛性焦慮障礙患者存在情緒語義無意識(shí)加工的異常。 2.兩組被試對(duì)情緒語義信息的無意識(shí)加工存在加工偏側(cè)化,即左半球額區(qū)為漢字情緒語義加工的主要部位。
[Abstract]:Objective: Generalized anxiety disorder (GAD), also known as chronic anxiety, is characterized by excessive and uncontrollable anxiety in daily events or activities. Although the current research on the pathogenesis of generalized anxiety disorder has not been consistent, the information processing theory of generalized anxiety disorder And the behavior, the imaging results suggest that automatic processing of emotional information or unconscious processing abnormalities may be one of the important pathogenesis of generalized anxiety disorder.
Information processing is characterized by no or minimal awareness of the process of information processing, which does not occupy the attention of resources and does not need awareness. Subliminal start - up paradigm is a widespread experimental paradigm for the study of unconscious processing. Thinking about the adverse consequences of certain events. Therefore, this experiment uses the subliminal affective priming paradigm. The event-related potentials (ERP) technology can make time accurate to milliseconds, and can dynamically reflect the time process of brain neuropsychological activity. It is the best technique to study the unconscious processing. The purpose of this study is to explore the characteristics of emotional semantic unconscious processing in patients with generalized anxiety disorder by using the event related potential method, and to further analyze the different emotional content and the distribution of the brain region.
Methods: the subjects included 12 patients with generalized disorder and 12 normal controls of age, sex and education. All subjects were strictly screened by standardized structural interview procedure (SCID), and the Hamilton Anxiety Scale (HAMA) and 17 Hamilton Depression Scale (HAMD-17) scores were completed. The emotional starting words are divided into three types of negative, positive and neutral emotions. The experimental task is to score the emotional grade of neutral pictures. The emotional start words and task pictures are selected from the Chinese emotional material database. The 64 Neuroscan electroencephalogram is used to record the event related potential. Then, the record is recorded. The EEG data were analyzed off-line. The average amplitude and incubation period of P1 and P3 components induced by different emotional priming words in the frontal area, the top area, the pillow area and the central region were measured respectively. The average amplitude and latency of the two groups of ERP components were analyzed by repeated measurement of variance analysis (Repeated measure ANOVA).
