【摘要】：目的研究老年患者術(shù)后認知功能障礙(postoperative cognitive dysfunction,POCD)情況及其對簡易精神狀態(tài)評價量表(MMSE)評分的影響。方法 2012年7月至2013年7月,該院共有156例老年患者納入研究,根據(jù)POCD診斷結(jié)果分為POCD組(54例)及無POCD組(102例)。整理并分析患者資料,分析POCD危險因素,對比兩組MMSE評分情況。結(jié)果POCD組在年齡、教育時限、合并高血壓、吸入麻醉藥物以及術(shù)后鎮(zhèn)痛等方面與無POCD組相比,差異均有統(tǒng)計學意義(P0.05)。Logistic回歸分析顯示,年齡、合并高血壓以及術(shù)后鎮(zhèn)痛為POCD疾病的危險因素。POCD組治療后的MMSE評分顯著低于治療前及無POCD組治療后的評分水平,差異均有統(tǒng)計學意義(均P0.05)。結(jié)論老年患者手術(shù)后POCD疾病的產(chǎn)生通常和年齡、合并高血壓以及術(shù)后鎮(zhèn)痛等危險因素有關(guān),臨床可通過MMSE評分量表對患者加以評測,值得臨床推薦。
[Abstract]:Objective to study the effect of postoperative cognitive dysfunction POCDD on the MMSE score in elderly patients after operation. Methods from July 2012 to July 2013, 156 elderly patients were included in the study. According to the diagnostic results of POCD, 54 cases were divided into POCD group (n = 54) and no POCD group (n = 102). Data of patients were analyzed and POCD risk factors were analyzed. MMSE scores of two groups were compared. Results the age, education duration, hypertension, inhaled anesthetic and postoperative analgesia in POCD group were significantly higher than those in no POCD group (P 0.05). Logistic regression analysis showed that age was significantly higher than that in POCD group. The risk factors of POCD disease were hypertension and postoperative analgesia. The MMSE scores of POCD group after treatment were significantly lower than those before treatment and without POCD group (all P 0.05). Conclusion the occurrence of POCD in elderly patients after operation is usually related to age, hypertension and postoperative analgesia, and can be evaluated by MMSE scale, which is worthy of clinical recommendation.
