【摘要】：目前經(jīng)顱磁刺激(repetitive transcranial magnetic stimulation)對(duì)阿爾茨海默病(Alzheimer's disease)的療效具有爭(zhēng)議性。檢索2015年12月以前Embase、medline、Cochrane Library、Ovid、Pubmed、Web of science、中國(guó)知網(wǎng)、萬(wàn)方醫(yī)學(xué)網(wǎng)數(shù)據(jù)庫(kù),按照既定的納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)查找rTMS治療AD的臨床隨機(jī)對(duì)照試驗(yàn)。使用Rev Man5.3軟件,應(yīng)用標(biāo)準(zhǔn)誤均數(shù)差(standardised mean difference),95%可信區(qū)間(confidence interval)分析。共納入5個(gè)隨機(jī)對(duì)照試驗(yàn)進(jìn)行meta分析,共223例患者,其中rTMS組137例,對(duì)照組86例,MMSE結(jié)果顯示rTMS組療效線顯著優(yōu)于對(duì)照組,(SMD=1.05,95%CI:0.21、1.88,p=0.01,n=141),亞組分析顯示高頻組(1 Hz)(p=0.02)療效優(yōu)于低頻組(≤1 Hz)(p=0.27);ADAS-cog結(jié)果顯示rTMS治療顯著優(yōu)于對(duì)照組,(SMD=-0.71,95%CI:-1.04、-0.39,p0.0001,n=168),高頻組(1 Hz)(p0.001)比低頻組(≤1 Hz)(p=0.05)療效更佳。分析結(jié)果顯示rTMS對(duì)AD認(rèn)知功能有顯著改善效果,且高頻治療效果優(yōu)于低頻治療,然而僅納入5個(gè)研究,后期需要增大隨機(jī)對(duì)照試驗(yàn)樣本量驗(yàn)證上述結(jié)論。
[Abstract]:The efficacy of transcranial magnetic stimulation (TMS) in Alzheimer's disease is controversial. To search for the database of Embase medline and Cochrane Library Ovidna Pubmedan Web of Science, China Zhiwang, Wanfang Medical Network before December 2015, and to search for clinical randomized controlled trials of AD treated with rTMS according to established inclusion criteria and exclusion criteria. Rev Man5.3 software is used to analyze 95% confidence intervalency of 95% confidence interval. Meta analysis was carried out in 5 randomized controlled trials, including 137 patients in rTMS group.
