【摘要】：目的分析精神分裂癥患者臨床特征對(duì)住院天數(shù)的影響,探討患者住院天數(shù)的預(yù)測(cè)指標(biāo),為臨床診療及預(yù)后提供參考依據(jù)。方法在廣州市腦科醫(yī)院精神科納入236例精神分裂癥患者,入院1周內(nèi)及出院前1周采用陽(yáng)性與陰性癥狀量表(positive and negative symptom scale,PANSS)、自知力和治療態(tài)度問(wèn)卷(insight andtreatment attitudes questionnaire,ITAQ)及藥物態(tài)度清單(drug attitude inventory,DAI)分別評(píng)估患者精神癥狀、自知力及服藥態(tài)度。分析住院天數(shù)與社會(huì)人口學(xué)資料、入院時(shí)的PANSS分、ITAQ分、DAI分,以及各量表出入院分?jǐn)?shù)差值的殘差之間關(guān)系。結(jié)果患者病程(r=0.199,P=0.002)和入院時(shí)PANSS陰性癥狀分(r=0.180,P=0.006)分別與住院天數(shù)呈正相關(guān)。線性回歸分析顯示病程(β=0.010,P=0.030)及入院時(shí)PANSS陰性癥狀分(β=0.286,P=0.010)與住院天數(shù)相關(guān)聯(lián)。結(jié)論病程、入院時(shí)PANSS陰性癥狀水平是精神分裂癥患者住院天數(shù)長(zhǎng)短的臨床預(yù)測(cè)指標(biāo),盡早采取積極的綜合治療方法以改善患者陰性癥狀對(duì)縮短住院天數(shù)有重要意義。
[Abstract]:Objective to analyze the influence of clinical characteristics of schizophrenia on the days of hospitalization, and to explore the predictors of the days of hospitalization in order to provide reference for clinical diagnosis and treatment and prognosis. Methods 236 schizophrenic patients were enrolled in the Department of Psychiatry, Guangzhou brain Hospital. Within 1 week of admission and 1 week before discharge, positive and negative symptom scale was used to evaluate the mental symptoms, insight and attitude questionnaire of insight andtreatment attitudes questionnaire, and drug attitude list (drug attitude inventory DAI) to evaluate the mental symptoms, insight and medication attitude of the patients. The results were as follows: (1) in the first week of admission and 1 week before discharge from hospital, the positive and negative symptom scale was used to evaluate the mental symptoms, insight and medication attitude of the patients, respectively. The relationship between the length of stay in hospital and the social demography data, the PANSS score and the Dai score at admission, and the residual difference between the scores of each scale were analyzed. Results there was a positive correlation between the duration of PANSS and the duration of the disease (0.199P0. 002) and the negative symptoms of PANSS (0. 180 P0. 006). Linear regression analysis showed that the course of disease (尾 -0.010) and the score of PANSS negative symptoms at admission (尾 -0.286) were correlated with the length of stay. Conclusion the course of disease and the level of PANSS negative symptoms at admission are clinical predictors of the length of stay in schizophrenia. It is important to take active comprehensive treatment to improve the negative symptoms in order to shorten the days of hospitalization.
