【摘要】：目的利用基于頭顱MRI的體素形態(tài)學分析(VBM)技術,觀察軀體形式疼痛障礙(SPD)患者腦灰質體積改變,并分析其與疼痛程度的關系。方法選擇SPD患者10例納入病例組、健康志愿者14例納入對照組。對SPD患者進行疼痛數(shù)字評價量表(NRS)評分。兩組均行頭顱MRI檢查,高分辨3D T1WI數(shù)據(jù)采集,保存圖像。將兩組頭顱MRI檢查所獲得的所有圖像數(shù)據(jù)進行VBM,測算全腦體積及灰質體積,并獲取兩組灰質體積差異有統(tǒng)計學意義的腦區(qū)。采用Pearson相關分析法分析SPD患者灰質體積與NRS評分的相關性。結果 VBM顯示,病例組灰質體積大于對照組(P0.01),灰質體積增加的區(qū)域主要位于左側丘腦。病例組左側丘腦灰質體積與NRS評分無顯著相關性(r=0.631,P0.05)。結論 SPD患者頭顱MRI VBM顯示腦灰質體積增大,主要位于左側丘腦;腦灰質體積改變與疼痛程度無明顯相關性。
[Abstract]:Objective to observe the changes of brain gray matter volume in patients with somatoform pain disorder (SPD) and to analyze the relationship between the volume of gray matter and the degree of pain by using the technique of morphologic analysis of voxel based on MRI. Methods Ten patients with SPD were selected to be included in the case group. 14 healthy volunteers were included in the control group. The patients with SPD were assessed with digital pain assessment scale (NRS). The head MRI was performed in both groups, and 3D T1WI data acquisition was performed in both groups. Save the image. The whole brain volume and gray matter volume were measured by using VBMs from all the image data obtained from the two groups of cranial MRI. Pearson correlation analysis was used to analyze the correlation between gray matter volume and NRS score in patients with SPD. Results VBM showed that there were significant differences in gray matter volume between the two groups. The gray matter volume in the case group was larger than that in the control group (P 0.01), and the area in which the gray matter volume increased was mainly located in the left thalamus. There was no significant correlation between the left thalamus gray matter volume and the NRS score in the case group. Conclusion the cranial MRI VBM of the patients with SPD shows the increase of the gray matter volume. It was mainly located in the left thalamus, and there was no significant correlation between the changes of gray matter volume and the degree of pain.
