【摘要】：目的:探討丁苯酞注射液治療老年急性腦梗死(ACI)患者的療效及其對(duì)腦血流動(dòng)力和腦血管儲(chǔ)備能力(CVR)的影響,闡明丁苯酞對(duì)ACI患者的藥理作用機(jī)制。方法:選取100例老年ACI患者作為研究對(duì)象,按入院病例號(hào)的單、雙號(hào)將其分為觀察組和對(duì)照組,每組各50例。對(duì)照組患者給予常規(guī)治療方法進(jìn)行治療,觀察組患者在常規(guī)治療方案的基礎(chǔ)上加用丁苯酞注射液進(jìn)行治療。觀察2組患者治療前后的美國國立衛(wèi)生研究院卒中量表(NIHSS)評(píng)分、雙側(cè)大腦中動(dòng)脈(MCA)的峰流速(Vp)、平均流速(Vm)及差值(DVp、DVm)等腦血流動(dòng)力學(xué)指標(biāo)和脈動(dòng)指數(shù)(PI)以及CVR,并進(jìn)行比較;對(duì)2組患者的臨床療效進(jìn)行評(píng)價(jià)和比較。結(jié)果:治療后觀察組患者NIHSS評(píng)分明顯低于對(duì)照組(t=15.420,P0.05),觀察組患者療效分布和臨床有效率均優(yōu)于對(duì)照組(U=2.225,χ~2=5.005,P0.05);觀察組患者治療后Vp和Vm明顯高于對(duì)照組(t=10.819,t=7.259,P0.05),而DVp和DVm明顯低于對(duì)照組(t=16.348,t=19.055,P0.05);觀察組患者治療后的CVR明顯高于對(duì)照組(t=6.884,P0.05),PI明顯低于對(duì)照組(t=4.979,P0.05)。結(jié)論:丁苯酞注射液可有效糾正ACI患者的腦血流動(dòng)力異常,提升機(jī)體穩(wěn)定腦血管灌注量的能力,改善ACI患者的神經(jīng)功能缺損癥狀,提高臨床療效。
[Abstract]:Objective: to investigate the efficacy of butylphthalide injection in the treatment of elderly patients with acute cerebral infarction (ACI) and its effect on cerebral hemodynamics and cerebral vascular reserve capacity (CVR), and to elucidate the pharmacological mechanism of butylphthalide on ACI patients. Methods: 100 elderly patients with ACI were divided into observation group (n = 50) and control group (n = 50) according to the single and double number of admission cases. The patients in the control group were treated with routine treatment, while those in the observation group were treated with butylphthalide injection on the basis of routine treatment.
