【摘要】：目的探討靜脈溶栓治療急性缺血性腦卒中時(shí)假卒中的臨床特征及其鑒別。方法回顧性分析第二軍醫(yī)大學(xué)長(zhǎng)海醫(yī)院2013年9月至2015年2月靜脈溶栓、明確診斷假卒中患者的臨床資料,并與缺血性腦卒中患者進(jìn)行比較,分析兩組患者臨床表現(xiàn)與實(shí)驗(yàn)室檢查結(jié)果的差異。結(jié)果靜脈溶栓患者212例,明確假卒中患者7例(3.3%),與缺血性腦卒中患者比較,兩組患者基線數(shù)據(jù)無(wú)明顯差異,精神心理障礙和癡呆病史對(duì)鑒別假卒中具有重要價(jià)值,頭顱MRI、血管評(píng)估、腦電圖、血液和腦脊液檢查有助于明確假卒中的最后診斷。結(jié)論少數(shù)靜脈溶栓治療的患者最后診斷為假卒中,患者病史結(jié)合頭顱MRI對(duì)鑒別診斷具有重要價(jià)值。
[Abstract]:Objective to investigate the clinical features and differential diagnosis of pseudoapoplexy in patients with acute ischemic stroke treated by intravenous thrombolysis. Methods the clinical data of intravenous thrombolysis in Changhai Hospital of the second military Medical University from September 2013 to February 2015 were analyzed retrospectively. the clinical data of patients with pseudo-stroke were clearly diagnosed and compared with those of patients with ischemic stroke. The differences between the clinical manifestations and laboratory results of the two groups were analyzed. Results compared with ischemic stroke patients, there was no significant difference in the baseline data between the two groups. Mental and psychological disorders and the history of dementia were of great value in distinguishing false stroke. MRI, vascular assessment, electroencephalogram (EEG), blood and cerebrospinal fluid examination were helpful to determine the final diagnosis of pseudopoplexy. Conclusion A small number of patients with intravenous thrombolysis are finally diagnosed as pseudopoplexy. The history of the patients combined with skull MRI is of great value in differential diagnosis.
