【摘要】：目的研究腦卒中患者在綜合醫(yī)院完成急性期治療后,后續(xù)康復(fù)機(jī)構(gòu)選擇及相關(guān)影響因素。方法回顧2015年1月至12月急性期腦卒中患者230例,探討年齡、首次出院時(shí)改良Rankin量表(m RS)評(píng)分、轉(zhuǎn)診機(jī)構(gòu)選擇等因素對(duì)轉(zhuǎn)診選擇的影響。結(jié)果共125例有轉(zhuǎn)診需求。m RS惡化,轉(zhuǎn)診需求增高(P0.05)。在轉(zhuǎn)診患者中,m RS 0~1分及2~3分患者對(duì)延續(xù)康復(fù)的需求高(F=4.300,P0.05);m RS 4~5分組對(duì)醫(yī)療質(zhì)量(F=3.464,P0.05)和對(duì)住院時(shí)長(zhǎng)要求高(F=3.692,P0.05)。≥80歲組對(duì)醫(yī)療質(zhì)量要求更高(F=4.630,P0.05)。在本院繼續(xù)治療者更關(guān)注延續(xù)康復(fù)治療(F=7.590,P0.001),選擇康復(fù)專(zhuān)科機(jī)構(gòu)者更關(guān)注康復(fù)質(zhì)量(F=32.156,P0.001),選擇社區(qū)和養(yǎng)老機(jī)構(gòu)者則更多考慮住院時(shí)長(zhǎng)(F=18.823,P0.001)和方便照料(F=14.070,P0.001)等。轉(zhuǎn)診醫(yī)療機(jī)構(gòu)的信息來(lái)源于醫(yī)生占72.0%,來(lái)源于患者占28.0%。結(jié)論腦卒中急性期患者的功能障礙程度、年齡、對(duì)最終預(yù)后的預(yù)期影響對(duì)轉(zhuǎn)診的選擇。醫(yī)方掌握更多的康復(fù)醫(yī)療資源,在轉(zhuǎn)診過(guò)程中可給患方提供更為合理的建議。
[Abstract]:Objective to study the choice of follow-up rehabilitation institutions and related influencing factors after acute stage treatment in general hospital for patients with cerebral apoplexy. Methods A total of 230 patients with acute stroke from January to December 2015 were reviewed. The influence of age, (m RS) score of modified Rankin scale on the first discharge and the selection of referral institution were investigated. Results there were 125 cases with.
