【摘要】：目的探討血栓彈力圖(TEG)預測老年重癥患者血栓發(fā)生的敏感度與特異度的臨床應用價值。方法選取該院重癥監(jiān)護室(ICU)于2015年10月至2016年12月收治的200例重癥患者進行回顧性分析,均接受TEG、常規(guī)凝血功能指標檢測及血小板計數(shù)(PLT)檢查,對TEG參數(shù)、凝血4項指標、PLT所得結果進行相關性分析,并對TEG、常規(guī)凝血功能指標預測敏感度、特異度進行評價。結果 TEG參數(shù)凝血反應時間(R)與凝血酶原時間(PT)無相關性,與活化部分凝血活酶時間(APTT)呈正相關,TEG參數(shù)凝血形成時間(K)與纖維蛋白原(Fib)、PLT呈負相關,TEG參數(shù)最大凝塊強度(MA)與Fib、PLT呈正相關,TEG參數(shù)凝固角(Angle)與Fib、PLT呈正相關(均P0.05);TEG參數(shù)R、K、MA、Angle與D-二聚體(D-dimer)均無相關性(P0.05);TEG參數(shù)R與K呈正相關,MA、Angle與K均呈負相關,MA與R呈負相關,MA與Angle呈正相關,Angle與R呈負相關(均P0.05);TEG、常規(guī)凝血功能指標預測血栓敏感度均50.0%,但TEG參數(shù)預測特異度高于常規(guī)凝血功能指標。結論 TEG預測老年重癥患者血栓發(fā)生的敏感度較低,特異度較高,能降低血栓發(fā)生誤診風險,具有一定指導意義,且與常規(guī)凝血功能指標無法相互替代,實際應用需與臨床、實驗室檢測結合進行綜合考慮。
[Abstract]:Objective To investigate the clinical value of thromboelastography (TEG) in predicting the sensitivity and specificity of thrombosis in elderly critically ill patients.Methods 200 critically ill patients admitted to intensive care unit (ICU) from October 2015 to December 2016 were retrospectively analyzed. Results There was no correlation between TEG parameters, prothrombin time (PT), activated partial thromboplastin time (APTT), TEG parameters and coagulation formation time (PT). The maximum coagulation strength (MA) of TEG parameters was positively correlated with Fib and PLT, and the coagulation angle (Angle) of TEG parameters was positively correlated with Fib and PLT (all P 0.05); the TEG parameters R, K, MA, Angle and D-dimer (D-dimer) were not negatively correlated (P 0.05); the TEG parameters R and K were positively correlated, MA, Angle and K were negatively correlated, MA and MA and R were negatively correlated. MA was positively correlated with Angle, Angle was negatively correlated with R (all P It is of guiding significance and can not be substituted with conventional coagulation function indexes. The practical application should be considered comprehensively in combination with clinical and laboratory tests.
