【摘要】：目的觀察膿毒癥多臟器功能障礙綜合征(MODS)患者血必凈注射液聯(lián)合氫化可的松治療前后血清單核細胞表面人類白細胞抗原DR位點(HLA-DR)和過氧化脂質(zhì)(LPO)水平變化情況。方法將236例膿毒癥MODS患者隨機分為對照組(78例)、觀察組1(78例)、觀察組2(80例),對照組給予綜合治療,觀察組1在對照組基礎上應用血必凈注射液治療,觀察組2在觀察組1治療基礎上應用氫化可的松治療。檢測各組治療前及治療后第1、3和7天血清HLA-DR和LPO。比較各組治療7 d內(nèi)病死率。結(jié)果治療后第3、7天,與對照組相比,觀察組1、觀察組2血清HLA-DR水平高,LPO水平低(P均0.05)。治療后第3、7天,與觀察組1相比,觀察組2血清HLA-DR水平高,LPO水平低(P均0.05)。對照組死亡48例,觀察組1死亡32例,觀察組2死亡20例,觀察組1、觀察組2與對照組相比,觀察組2與觀察組1相比,P均0.05。結(jié)論膿毒癥MODS患者血必凈注射液聯(lián)合低劑量氫化可的松治療后血清HLA-DR水平升高,LPO水平降低。
[Abstract]:Objective to observe the changes of HLA-DR and LPO on the surface of monocytes in patients with septic multiple organ dysfunction syndrome (mods) before and after treatment with Xuebijing injection and hydrocortisone. Methods 236 patients with sepsis mods were randomly divided into control group (78 cases), observation group 1 (78 cases) and observation group 2 (80 cases). Observation group 2 was treated with hydrocortisone on the basis of observation group 1. Serum HLA-DR and LPO were measured before treatment and on the 1st and 7th day after treatment. The mortality of each group within 7 days was compared. Results compared with the control group, the serum HLA-DR level in the observation group 1 was higher than that in the control group (P 0.05), and the serum HLA-DR level in the observation group 2 was higher than that in the control group (P 0.05). On the 3rd day after treatment, the serum HLA-DR level in the observation group was higher than that in the observation group 1 (P 0.05). 48 cases died in the control group, 32 cases died in the observation group 1, 20 cases died in the observation group 2, the observation group 1, the observation group 2 compared with the control group, the observation group 2 compared with the observation group 1 (P 0.05). Conclusion the serum HLA-DR level in septic mods patients increased and LPO decreased after treatment with Xuebijing injection and low dose hydrocortisone.
