【摘要】：目的：探討胸腔鏡輔助下Nuss手術(shù)治療先天性漏斗胸的療效和安全性。 方法：2005年7月至2013年4月，對(duì)131例年齡3-21歲（平均13.6歲）先天性漏斗胸（對(duì)稱型73例，非對(duì)稱型58例）患者實(shí)施胸腔鏡輔助下Nuss手術(shù)治療。 結(jié)果：131例患者均順利完成手術(shù)，無(wú)死亡、大出血及胸腔臟器損傷等危險(xiǎn)并發(fā)癥發(fā)生。術(shù)中出血量3-22ml，平均8.5ml；手術(shù)時(shí)間40-89min，平均53min；術(shù)后住院天數(shù)4-9d，平均6.5d。術(shù)后并發(fā)癥包括8例氣胸、3例胸腔積液、2例皮下氣腫、2例鋼板移位、1例鋼板過(guò)敏反應(yīng)。本組中所有患者計(jì)劃于術(shù)后2年拆除鋼板，目前已有88例患者按計(jì)劃于術(shù)后2年順利拆除鋼板，其中74例恢復(fù)至正常胸部解剖，12例存在輕微的胸部凹陷，2例手術(shù)效果一般，優(yōu)良率為97.7％。 結(jié)論：胸腔鏡輔助下Nuss手術(shù)治療先天性漏斗胸具有創(chuàng)傷小、出血少、不切骨、恢復(fù)快、效果滿意，方法安全可靠等優(yōu)點(diǎn)，可廣泛應(yīng)用于漏斗胸的治療。
[Abstract]:Objective: to evaluate the efficacy and safety of thoracoscopic Nuss operation in the treatment of congenital funnel chest. Methods: from July 2005 to April 2013, 131 patients aged 3-21 years (mean 13.6 years) with congenital funnel chest (73 symmetric and 58 asymmetric) underwent thoracoscopic Nuss surgery. Results all the 131 cases were successfully operated without death, massive hemorrhage, thoracic organ injury and other dangerous complications. The intraoperative bleeding volume was 3-22 ml (mean 8.5 ml), the operative time was 40-89 min (mean 53 min), and the postoperative hospital stay was 4-9 d (mean 6.5 d). Postoperative complications included 8 cases of pneumothorax, 3 cases of pleural effusion, 2 cases of subcutaneous emphysema, 2 cases of plate transposition and 1 case of plate hypersensitivity. All the patients planned to remove the plate 2 years after operation. At present, 88 cases have successfully removed the plate 2 years after the operation, of which 74 cases returned to normal thoracic anatomy in 12 cases, and 2 cases had mild chest depression. The excellent and good rate is 97. 7%. Conclusion: the treatment of congenital funnel chest with thoracoscopic assisted Nuss operation has the advantages of less trauma, less bleeding, no osteotomy, rapid recovery, satisfactory effect, safe and reliable, and can be widely used in the treatment of funnel chest.
