【摘要】：目的探討影響重型顱腦損傷(TBI)去骨瓣減壓術(DC)預后的因素。方法分析112例TBI的臨床資料,采用格拉斯哥預后評分(GOSE)評估預后。評估的變量包括格拉斯哥昏迷評分(GCS)、入院瞳孔變化、中線結構移位、顱內病變數(shù)和入院-手術時間。結果傷后3個月病死率為38.4%。GCS 4~6分、瞳孔散大、中線結構移位超過15 mm患者與預后不良密切相關(P0.05),入院-手術時間和顱內病變數(shù)與預后不良無明顯相關。結論行DC的TBI中,年齡≥65歲、GCS 4~6分、瞳孔散大、中線結構移位超過15 mm患者預后更差。
[Abstract]:Objective to investigate the prognostic factors of craniocerebral injury with craniocerebral injury (TBI). Methods the clinical data of 112 patients with TBI were analyzed and the prognosis was evaluated by Glasgow prognostic score (Glasgow). Variables evaluated included Glasgow coma scale GCSA, pupil changes, midline structural shift, number of intracranial lesions, and hospital-operation time. Results the fatality rate of 3 months after injury was 38.4%.GCS 4 ~ 6, pupil dilated, median structure shift more than 15 mm was closely related to poor prognosis (P0.05), hospitalization-operation time and the number of intracranial lesions had no significant correlation with poor prognosis.
