【摘要】：目的:探討丹參滴丸聯(lián)合強(qiáng)化降脂治療對急性心肌梗死PCI術(shù)后再梗死的預(yù)防作用。方法:選取醫(yī)院自2014年4月—2014年11月確診的急性ST段抬高型心肌梗死行PCI術(shù)患者104例,根據(jù)隨機(jī)數(shù)字表法進(jìn)行分組,對照組52例予以術(shù)后予以常規(guī)降脂治療,控制原發(fā)病等常規(guī)治療,研究組52例在常規(guī)治療上予以丹參滴丸(10丸/次,tid)聯(lián)合強(qiáng)化降脂(阿托伐他汀鈣片80 mg/d,qd)治療,3個(gè)月為1療程。用藥前、介入后24 h及介入后7 d采血測定血清細(xì)胞因子、心肌損傷標(biāo)志物及血管內(nèi)皮功能指標(biāo),對比兩組間心肌微循環(huán)灌注狀況,進(jìn)行2年的隨訪,觀察并記錄心血管事件。結(jié)果:經(jīng)單因素方差分析,與用藥前比較,兩組用藥不同時(shí)間點(diǎn)血清細(xì)胞因子、心肌損傷及血管內(nèi)皮功能指標(biāo)存在差異(P0.05);與對照組比較,研究組介入后24 h、介入后7 d時(shí)血清CRP、TNF-α、IFN-γ、IL-6含量較低,血清CK-MB、c Tn I、BNP含量較低,血清ET、s ICAM-1含量較低,NO含量較高(P0.05);研究組用藥后心肌微循環(huán)灌注改善程度更高(P0.05);隨訪2年期間無病例脫落現(xiàn)象,兩組間心血管事件對比,無差異(P0.05)。結(jié)論:丹參滴丸聯(lián)合強(qiáng)化降脂治療對急性心肌梗死PCI術(shù)后心肌保護(hù)作用較強(qiáng),抑制手術(shù)導(dǎo)致的炎癥反應(yīng)及血管內(nèi)皮損傷,改善心肌微循環(huán)灌注。
[Abstract]:Objective: to investigate the preventive effect of Danshen dropping pill combined with enhanced lipid-lowering therapy on reinfarction after acute myocardial infarction (PCI). Methods: 104 patients with acute ST-segment elevation myocardial infarction diagnosed from April 2014 to November 2014 in our hospital were treated with PCI. According to the random digital table method, 52 patients in the control group were treated with conventional lipid-lowering therapy and control of primary disease. 52 patients in the study group were treated with Danshen dropping pills 10 pills / times tid combined with enhanced lipid lowering (80 mg / d QD of Atto vastatin calcium tablets) for 3 months. Serum cytokines were measured 24 hours before intervention 24 hours after intervention and 7 days after intervention. Myocardial injury markers and vascular endothelial function indexes were compared between the two groups, followed up for 2 years, cardiovascular events were observed and recorded. There were significant differences in serum cytokines, myocardial injury and vascular endothelial function between the two groups at different time points (P 0.05), compared with the control group, the levels of serum CRPn- TNF- 偽 IFN- 緯 IL-6 and serum CK-MBC Tn I BNP in the study group were lower than those in the control group at 24 hours after intervention and 7 days after intervention. The content of serum ETS ICAM-1 was lower and the content of no was higher than that of control group (P 0.05). The myocardial microcirculation perfusion in the study group was improved significantly (P 0.05). There was no case abscission during the follow-up of 2 years, and the cardiovascular events were compared between the two groups. Conclusion: Salvia miltiorrhiza drop pill combined with enhanced lipid-lowering therapy has a strong protective effect on myocardium after acute myocardial infarction (PCI). It can inhibit the inflammatory reaction and vascular endothelial injury induced by operation and improve myocardial microcirculation perfusion.
