S100A12在急性呼吸窘迫綜合征患者外周血中的表達(dá)及意義

【摘要】:目的探討S100A12蛋白水平在急性呼吸窘迫綜合征(ARDS)患者外周血中的表達(dá)及臨床意義。方法納入濰坊醫(yī)學(xué)院附屬醫(yī)院、濰坊市人民醫(yī)院2015年10月至2016年12月收治的ARDS患者,根據(jù)患者氧合狀態(tài)選取輕度組(n=20)、中度組(n=20)和重度組(n=20),共60例納入研究。隨訪患者30d預(yù)后,情況,分為生存組(n=37)和死亡組(n=23)。選取同期60例健康體檢者作為對(duì)照組。收集所有研究對(duì)象臨床基本資料。采集所有研究對(duì)象清晨空腹靜脈血4ml,采用ELISA方法檢測(cè)血清中S100A12的表達(dá)。結(jié)果與對(duì)照組比較,ARDS患者血清中S100A12、白細(xì)胞介素-1(IL-1)、腫瘤壞死因子-α(TNF-α)水平以及急性生理學(xué)和慢性健康狀況評(píng)分系統(tǒng)Ⅱ(APACHEⅡ)評(píng)分顯著升高,PaO_2/FiO_2明顯降低,差異有統(tǒng)計(jì)學(xué)意義(均P0.05)。S100A12水平與ARDS嚴(yán)重程度呈正相關(guān),重度組患者血清中S100A12水平高于中度組,中度組高于輕度組,差異有統(tǒng)計(jì)學(xué)意義(均P0.05)。ARDS患者血清中S100A12水平與IL-1、TNF-α、APACHEⅡ評(píng)分呈正相關(guān)(P0.05),而與PaO_2/FiO_2呈負(fù)相關(guān)(P0.05)。對(duì)死亡組有關(guān)影響因素進(jìn)行Logistic回歸分析顯示IL-1、TNF-α、APACHEⅡ評(píng)分、S100A12是ARDS患者死亡的獨(dú)立危險(xiǎn)因素,PaO_2/FiO_2則是ARDS患者死亡的獨(dú)立保護(hù)因素。ARDS患者血清中S100A12蛋白水平在預(yù)后評(píng)估時(shí)的ROC曲線下面積表明S100A12蛋白水平對(duì)ARDS的轉(zhuǎn)歸有一定的預(yù)測(cè)價(jià)值,可作為預(yù)后評(píng)估的輔助性指標(biāo)。結(jié)論 ARDS患者血清中S100A12水平顯著升高,與病情嚴(yán)重程度密切相關(guān),對(duì)ARDS早期診斷、治療及預(yù)后有著一定的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to investigate the expression of S100A12 protein in peripheral blood of patients with acute respiratory distress syndrome (ARDS) and its clinical significance. According to the oxygenation status of the patients, 60 patients were included in the study, who were selected as mild group, moderate group, and severe group, 60 cases were included in the study. The prognosis of the patients was followed up for 30 days. The patients were divided into survival group (n = 37) and death group (n = 23). 60 healthy persons were selected as control group. Basic clinical data of all subjects were collected. Fasting venous blood was collected from all subjects in the morning. Serum S100A12 was detected by ELISA method.
