【摘要】：目的探討膿毒血癥引起肺損傷的相關機制。方法將30只SD大鼠隨機均分為正常對照組和模型組,1周后分別予等體積的生理鹽水、8 mg/kg的脂多糖(LPS)尾靜脈注射,48 h后取支氣管肺泡灌洗液(BALF)和肺組織,測定BALF中的中性粒細胞計數(shù);ELISA法測定肺組織和BALF中腫瘤壞死因子-α(TNF-α)及白細胞介素-6(IL-6)的濃度,稱重并計算肺組織濕/干重比(W/D)。觀察小鼠肺組織病理切片并行肺損傷評分。結果與正常對照組比較,模型組小鼠肺泡組織結構發(fā)生明顯改變,大鼠肺組織腫大,有輕微的潰爛,且肺組織W/D較正常對照組顯著增加,HE染色顯示模型組組織膨脹,細胞水腫疏松,并有部分細胞脫落,肺損傷評分明顯升高(P0.01),正常對照組無明顯病變。模型組BALF中的中性粒細胞計數(shù)顯著提高(P0.01),肺組織和BALF中TNF-α和IL-6濃度顯著升高(P0.01)。結論膿毒血癥肺組織、BALF炎性因子TNF-α和IL-6明顯升高,膿毒血癥所致急性肺損傷的病變程度加重,中性粒細胞的浸潤、肺組織炎癥介質(zhì)的產(chǎn)生和滲出增加。膿毒血癥可引起的肺水腫和大分子物質(zhì)滲出。
[Abstract]:Objective to investigate the mechanism of lung injury induced by sepsis. Methods Thirty SD rats were randomly divided into normal control group and model group after injection of the same volume of normal saline for 8 mg/kg of lipopolysaccharide (LPS) into tail vein for 48 h. Bronchoalveolar lavage fluid (BALF) and lung tissue, The concentrations of tumor necrosis factor- 偽 (TNF- 偽) and interleukin-6 (IL-6) in lung tissue and BALF were determined by Elisa. The lung tissue wet / dry weight ratio was measured and the lung tissue pathological sections were observed with lung injury score. Results compared with the normal control group, the alveolar tissue structure of the model group was obviously changed, the lung tissue in the model group was enlarged, and the lung tissue was slightly ulcerated. Compared with the normal control group, the lung tissue W- / D increased significantly with HE staining, which showed that the model group had tissue swelling, cell edema and loose, and some of the cells were exfoliated. In model group, the neutrophil count in BALF and the concentrations of TNF- 偽 and IL-6 in lung tissue and BALF were significantly increased (P 0.01). Conclusion the inflammatory cytokines TNF- 偽 and IL-6 in septic lung tissue were significantly higher than those in control group. The severity of acute lung injury due to sepsis is aggravated, neutrophils infiltrate, and the production and exudation of inflammatory mediators in lung tissue are increased.
