【摘要】:目的:觀察風(fēng)濕免疫相關(guān)性血細(xì)胞減少(Rheumatism immune-related hematocytopenia,RIRH)患者骨髓免疫細(xì)胞炎性因子分泌狀態(tài),探討其對(duì)骨髓造血細(xì)胞損傷的病理作用及臨床意義。方法:①ELISA法檢測(cè)66例RIRH患者血清IL-5、IL-10、IL-12、IL-17和IFN-γ濃度,對(duì)照組30例采自健康查體血清;免疫化學(xué)法觀察骨髓免疫細(xì)胞HLA-DR表達(dá);②免疫熒光法觀察骨髓細(xì)胞抗人球蛋白(IgG)抗體、FcγⅡ受體(FcγⅡR)、甘露糖受體(MR)、ICAM-1、IL-12、IL-17A與IL-17RA表達(dá);30例缺鐵性貧血患者骨髓作為疾病對(duì)照;③隨機(jī)抽取32例血小板50×109L-1無(wú)出血傾向患者,常規(guī)治療同時(shí)聯(lián)合復(fù)方丹參促進(jìn)炎性因子代謝,與30例不聯(lián)合復(fù)方丹參患者對(duì)照治療效果。結(jié)果:①66例患者血清IL-5、IL-10、IL-12、IL-17和IFN-γ濃度分別為(126.04±7.66)ng/L,(316.23±25.163)ng/L,(328.43±27.01)ng/L,(48.17±4.22)ng/L和(40.28±3.69)ng/L,較之對(duì)照組(33.16±2.17)ng/L,(176.25±10.69)ng/L,(94.05±3.26)ng/L,(20.03±1.14)ng/L和(8.25±2.01)ng/L顯著升高;②骨髓中活化的組織嗜堿細(xì)胞、嗜酸性粒細(xì)胞(EOS)、樹(shù)突狀細(xì)胞和巨噬細(xì)胞(Mφ)等免疫細(xì)胞高表達(dá)HLA-DR、FcγⅡR、MR、ICAM-1、IL-12、IL-17A和IL-17RA;③32例IRHS患者聯(lián)合復(fù)方丹參組經(jīng)治療2周后,血清IL-5和IL-17含量較之對(duì)照組顯著降低,血象恢復(fù)時(shí)間較之對(duì)照組提前1周。結(jié)論:RIRH患者骨髓中多種免疫細(xì)胞活化,表達(dá)和分泌多種免疫分子,介導(dǎo)ADCC效應(yīng)、細(xì)胞免疫功能和炎性反應(yīng),是損傷病變的造血細(xì)胞的重要免疫機(jī)制?垢腥、抑制免疫聯(lián)合復(fù)方丹參活血化瘀,能改善骨髓造血微環(huán)境,促進(jìn)造血恢復(fù)。
[Abstract]:Objective: To observe the immune related hematocytopenia rheumatism (Rheumatism immune-related, hematocytopenia, RIRH) in patients with bone marrow cells secretion of inflammatory factors, and explore the pathological role of bone marrow hematopoietic cell injury and its clinical significance. Methods: detection of 66 cases by ELISA RIRH in sera of patients with IL-5, IL-10, IL-12, IL-17 concentration and IFN- gamma, control group of 30 cases collected from healthy serum; immunohistochemical method was used to observe the expression of HLA-DR in bone marrow cells; bone marrow cells were observed by immunofluorescence anti human globulin (IgG) antibody, Fc receptor gamma (Fc gamma R), mannose receptor (MR), ICAM-1, IL-12, IL-17RA and IL-17A expression in 30 cases; iron deficiency anemia in patients with bone marrow as a disease control; the randomly selected 32 cases of platelet 50 * 109L-1 patients with bleeding tendency, and routine treatment combined with compound salvia miltiorrhiza promotes inflammatory factor and metabolism, 30 cases of patients treated with Compound Danshen control 鐤楁晥鏋,
