鮑氏不動桿菌性呼吸機(jī)相關(guān)性肺炎的臨床預(yù)測因素分析

【摘要】:目的探討急診重癥監(jiān)護(hù)病房(EICU)呼吸機(jī)相關(guān)性肺炎(VAP)患者中鮑氏不動桿菌感染的臨床預(yù)測因素,以期提高預(yù)防意識,降低其感染發(fā)生率。方法回顧性分析2009年7月-2015年6月243例入住EICU的VAP患者臨床資料,數(shù)據(jù)采用SPSS 18.0軟件進(jìn)行統(tǒng)計分析。結(jié)果 243例VAP患者發(fā)生鮑氏不動桿菌性呼吸機(jī)相關(guān)性肺炎(AB-VAP)54例,發(fā)生率為22.22%;單因素分析發(fā)現(xiàn),多種因素有預(yù)測AB-VAP價值;多因素logistic回歸分析發(fā)現(xiàn),機(jī)械通氣時間≥15d、急性生理及慢性健康評分≥21、糖皮質(zhì)激素的使用及氣管切開具有獨(dú)立預(yù)測價值。結(jié)論長時間機(jī)械通氣、急性生理及慢性健康Ⅱ評分高、使用激素及氣管切開為EICU VAP患者發(fā)生鮑氏不動桿菌感染的獨(dú)立預(yù)測因素,盡早脫機(jī)、加強(qiáng)營養(yǎng)支持治療、謹(jǐn)慎使用激素、嚴(yán)把氣管切開指征等措施有可能降低AB-VAP的發(fā)生。
[Abstract]:Objective to investigate the clinical predictors of Acinetobacter baumannii infection in (VAP) patients with (EICU) ventilator-associated pneumonia in emergency intensive care unit (ICU) in order to improve the awareness of prevention and reduce the incidence of Acinetobacter baumannii infection. Methods the clinical data of 24 VAP patients admitted to EICU from July 2009 to June 2015 were analyzed retrospectively. the data were statistically analyzed by SPSS 18.0 software. Results 54 patients (22.22%) developed Acinetobacter baumannii ventilator-associated pneumonia (AB-VAP) in 24 VAP patients, and univariate analysis showed that many factors were valuable in predicting AB-VAP.
